O N T R A C T. This Contract, effective the last date signed below, is by and between the State of Florida, Department of Management Services (“Department”), an agency of the State of Florida with offices at 0000 Xxxxxxxxx Xxx, Xxxxxxxxxxx, Xxxxxxx 00000-0000, and the entity identified below as Contractor (“Contractor”). The Contractor responded to the Department’s Invitation to Bid (“ITB”) No. 18-863-000-S; Tires. The Department has determined to accept Contractor’s offer and to enter into this Contract in accordance with the specifications, requirements, terms, and conditions of the solicitation. The specific products and services awarded to Contractor, and the maximum rates Contractor may charge Eligible Users, are identified on the attached Price Sheet & Ordering Instructions, Section 7.1. Accordingly, and in consideration of the mutual promises contained in the Contract documents, the Department and the Contractor do hereby enter into this Contract, which is a state term contract authorized by section 287.042(2)(a) of the Florida Statutes (2001). The term of the Contract begins on the Contract Formation Date, per Section 2.17, and expires February 29, 2012. O N T R A C T. The contract is subject to revision at any time, upon request by either the professional supervisor or supervisee. The contract will be reviewed each 6 months on the approval of both the professional supervisor and the supervisee. The price per one-hour session will be $155 gst inclusive, or $80 for student supervisees. Invoices with amount owing are to be sent to the supervisee, with payment due 2 days prior to the session. Proof of insurance will be arranged by supervisee: Yes No If yes, proof of such must be provided as soon as possible. Date proof provided, with copy to supervisor Professional membership details to be provided by supervisee for search by supervisor. If not yet a member of a professional body, information to be provided by supervisee once obtained. Date provided Sessions are to be made by appointment only. Appointments can be made by phone or via online booking system, but only with assigned Supervisor.
