Operating Engineers Health and Welfare Trust Combined Medical and Dental Insurance Plan Options. For coverage effective February 1, 2017 through January 31, 2018, in lieu of County offered medical and dental plans, an employee may enroll in an Operating Engineers Health and Welfare Trust medical and dental plan to the extent permitted by that Trust. For full-time employees, the County shall contribute towards that coverage in an amount not to exceed eighty- seven and a half percent (87.5%) of the total monthly premium, at the employee’s applicable level of enrollment, for the lowest cost HMO plan offered through the County plus the amount that the County would have contributed toward the County- offered dental plan had the employee enrolled in that plan, or the premium of the elected Operating Engineer’s plan, whichever is lower.

Operating Engineers Health and Welfare Trust Combined Medical and Dental Insurance Plan Options. For coverage effective January 18, 2015 through January 31, 2017, the County shall contribute the dollar equivalent of ninety percent (90%) of the total monthly premium of the lowest cost HMO plan offered by the County toward monthly premium for Operating Engineers Health and Welfare Trust Plan at the corresponding level of coverage (i.e. Self, Self + 1 dependent, Family). Because the Operating Engineers Health and Welfare Trust Plan includes dental coverage, the County shall also contribute the dollar equivalent of the premium for the County Delta Dental Premier Plan at the corresponding level of coverage (i.e. Self, Self + 1 dependent, Family) toward the monthly premium of the Operating Engineers Health and Welfare Trust Plan.

  • Dental Insurance Plan 9.9.1 The College will pay one hundred percent (100%) of the premiums for a dental insurance plan, except as per 9.1.4.1.

  • HEALTH AND LIFE INSURANCE The Sheriff will continue to offer to employees of this bargaining unit the same health and life insurance programs being offered to other Sheriff’s Office employees. Due to the changing nature of the health insurance market, the Sheriff retains the right during the term of this Agreement to develop plan changes or to change carriers in order to reduce costs or for other reasons. Changes to the level and types of benefits shall be subject to bargaining as provided by law. Rates paid by the employee will be established by the Sheriff.

  • Hospitals of Ontario Voluntary Life Insurance Plan The Hospital also agrees to make the Hospitals of Ontario Voluntary Life Insurance Plan (HOOVLIP) available to the nurses subject to the provisions of HOOVLIP at no cost to the Hospital.

  • Group Insurance Plan The carriers, coverage, and terms and conditions of participation under the District’s Group Insurance Plan are subject to change in accordance with the applicable provisions of Title I, Division 4, Chapter 10 of the California Government Code (Section 3500 et seq.)   • GROUP INSURANCE PLANS 15.01 The Company agrees to provide all full time employees with one (1) or more years of continuous service, a *Short Term Disability Benefit (S.T.D.) as generally described in Section B of a notice to all employees of the amended Benefit Plan dated May 1, 1993. *Payment for Short Term Disability shall begin on the third (3rd) consecutive day of absence. For those employees who have completed ninety (90) days of perfect attendance from the last date of return to work from an absence due to sickness or accident, shall be paid S.T.D. from the first (1st) day for the first covered absence following the ninety (90) days of perfect attendance.

  • Ontario Health Insurance Plan The parties recognize that the method of funding OHIP has been changed from an individually paid premium to a system funded by an employer paid payroll tax. If the government, at any time in the future, reverts to an individually paid premium for health insurance, the parties agree that the Colleges will resume paying 100% of the billed premium for employees.

  • Group Dental Insurance Not available to part-time Station Attendants. Group insurance coverage for temporary full-time employees will be in accordance with XXX #1. Such benefits, once established, are retained even if an employee's status reverts back to part-time, providing that employment has been continuous.
