Common use of Optical Coverage Clause in Contracts

Optical Coverage. Employees, their spouses and eligible dependents shall be provided vision coverage through Eye-Med Advantage as described in Appendix C, or its satisfactory equivalent. Vision benefits are available to dependents through the end of the calendar year in which they turn 19 or until age 25 if they are full time students. Proof of student status will be requested annually after the age of 19 to verify eligibility. Plan specifics are outline in Appendix C.
