Optional Supplemental Insurance. 37 38 An employee may elect to participate in optional group insurance programs 39 approved by the SEBB. The entire premium for any of these Optional 40 Supplemental Insurances will be paid by the employee, however employees will 41 be able to utilize payroll deduction for any supplemental insurance that they 42 choose to enroll in through SEBB (e.g., increased Life, AD&D, Long Term 43 Disability, etc.). 45 11.3 A District Advisory Insurance Committee may be convened to assist in 46 determining any allowable supplemental types of insurance programs to be 47 provided to employees subject to approval by the Board. If an advisory 1 committee is convened, the union shall be entitled to name one representative to 2 the committee.

Optional Supplemental Insurance. 38 39 An employee may elect to have optional group insurance programs approved by 40 the District (e.g. Supplemental Life, two times annual salary, American Fidelity 41 Short Term Disability Insurance and INA - High Limit Accident) deducted from 42 their salary warrant. The entire premium for any of these optional supplemental 43 insurances will be a full salary deduction. None of the unused contribution, from 44 the Basic Group Insurance as described above can be applied to the Optional 45 Supplemental Insurance premiums.

  • Group Dental Insurance Not available to part-time Cabin Servicing & Cleaning Attendants. Group insurance coverage for temporary full-time employees will be in accordance with XXX #1. Such benefits, once established, are retained even if an employee's status reverts back to part-time, providing that employment has been continuous.

  • Dental Insurance The State agrees to pay one hundred percent (100%) of the employee premium of a dental insurance program for full-time employees. The benefit levels of this program shall provide one hundred percent (100%) coverage for preventive care and eighty percent (80%) coverage for general service care.   • Health and Dental Insurance ☐ Husband ☐ Wife shall maintain coverage for each minor child under the medical and dental insurance provided through his/her employment. To facilitate the use of such coverage for the child(ren), the Couple shall cooperate fully and in a timely manner, including, but not limited to, obtaining and providing all necessary insurance cards and claim forms, completing and submitting all necessary documents, and delivering all insurance payments. For purposes of duration and modification, this provision shall be deemed part of the child support orders made by the local court in the Couples’ dissolution action.

  • Medical and Dental Insurance The Company shall pay Employee’s monthly Medical and Dental Insurance premiums in association with Company provided health insurance plans.

  • Certificates of Insurance Prior to commencing under this Contract, Supplier must furnish to Sourcewell a certificate of insurance, as evidence of the insurance required under this Contract. Prior to expiration of the policy(ies), renewal certificates must be mailed to Sourcewell, 000 00xx Xxxxxx Xxxxxxxxx, X.X. Box 219, Staples, MN 56479 or sent to the Sourcewell Supplier Development Administrator assigned to this Contract. The certificates must be signed by a person authorized by the insurer(s) to bind coverage on their behalf. Failure to request certificates of insurance by Sourcewell, or failure of Supplier to provide certificates of insurance, in no way limits or relieves Supplier of its duties and responsibilities in this Contract.

  • Optional Insurance Landlord may maintain such additional insurance with respect to the Building and the Complex, including, without limitation, earthquake insurance, terrorism insurance, flood insurance, liability insurance and/or rent insurance, as Landlord may in its sole discretion elect. Landlord may also maintain such other insurance as may from time to time be required by the holder of any mortgage on the Building or Property. The cost of all such additional insurance shall also be part of the Landlord’s Operating Expenses.

  • Additional Life Insurance All eligible employees shall be permitted to purchase term life insurance in addition to the amount provided by the Board in paragraph C. of this Article.
