Additionally, we will use a service called CSIeSafe to safely and securely deliver your electronic documents. CSIeSafe will store these documents for 90 days from the date of delivery. You may print or download your documents to retain copies of them. You may subscribe to CSIeSafe’s long term storage at an additional fee, if you would like for them to be available for a longer period. A file will be included with your electronic document delivery that will contain a list of all your banking transactions. That file can easily be imported to Comma separated (.csv), PDF Document (.pdf), Word Document (.doc), BAI2 File. Pop up blocker must be turned off for the CSIeSafe domain. JavaScript enabled.

You will receive notification through an e-mail from PTSB that your e-Statement(s) and e-Notice(s) is available for viewing and retention. This can only be delivered to one e-mail address. If the e-mail notification is returned as “undeliverable” we will print and mail a paper statement to your postal address that is on file with PTSB. This may also cause the electronic delivery to be removed from your account(s). When you provide PTSB with a valid e-mail address, you can resume electronic delivery.

Additionally, you will have access to your e-Statements through our Online Banking service for up to eighteen months from the date of delivery. After this, you must contact the Bank to get a paper copy. The fees and charges as outlined above for paper copies will apply. You may print or save your e-Statements to retain your copies. You must ensure that you safeguard your access and storage of electronic copies just as you would your paper copies to maintain privacy and security of your personal information.
Our Requirements. You and your attending Medical Practitioner must complete the claim form and return it to us. We may also ask you to provide us with, at your expense, additional information that we consider necessary to properly assess the claim and satisfy us of our liability to pay any benefit under this Policy. This may include an examination of your financial records and tax returns. If you are self-employed, a working director or a partner in a partnership, we may also examine the account- ing records of the business or practice if we consider this necessary to assess your claim. Payment of a benefit is subject to you undergoing any medical examination (by a Medical Practitioner of our choice) or other examination which we consider necessary, at our expense. We may also require you to provide us with your authority to obtain further medical information about you in the course of the claim.

In order to use the Internet Banking Service, you need a computer (in this Agreement, your computer and the related equipment are referred to together as your "Computer") with a web browser (either Firefox 3.0 or higher, Microsoft Internet Explorer 7.0 or higher) and Adobe Acrobat Reader 5.0 or greater installed. ❖ Third, if technical requirements needed to access the eStatements change after you have been receiving electronic documents, we will notify you of the new requirements. You have the right to withdraw consent to have records provided electronically. To discontinue this electronic delivery service, you may request a discontinuance of the service in writing to the Bank by mail or in person to any branch. The mailing address and branch locations for the Bank are set forth in this Consent and Agreement. It may take up to 45 days for the Bank to implement your request, and after such time you will no longer receive your statements electronically. We will charge no fees for discontinuing this service; however, a monthly service charge may apply for your type of checking account. ❖ Fourth, before you can begin receiving your eStatements, you will receive an email containing a PDF document. This is to verify you can view your statements in our Internet Banking. You will need to open the PDF file and view the word on the page.
