Physician and other professional services. Benefits are available for services performed by a Physician, surgeon, or other Health Care Provider to diagnose or treat a medical condition. Benefits include: • Office visits for examination, diagnosis, counseling, education, consultation, and treatment; • Specialist office visits; • Urgent care center visits; • Second medical opinions; • Administration of injectable medications; • Outpatient services; • Inpatient services in a Hospital, Skilled Nursing Facility, residential treatment center, or emergency room; • Home visits; • Telehealth consultations, provided remotely via communication technologies, for examination, diagnosis, counseling, education, and treatment; and • Teladoc primary care consultations. PKU formulas and special food products Benefits are available for formulas and special food products if you are diagnosed with phenylketonuria (PKU). The items must be part of a diet prescribed and managed by a Physician or appropriately-licensed Health Care Provider. Benefits include: • Enteral formulas; and • Special food products for the dietary treatment of PKU. Benefits do not include: • Grocery store foods used by the general population; or • Food that is naturally low in protein, unless specially formulated to have less than one gram of protein per serving. Podiatric services Benefits are available for the diagnosis and treatment of conditions of the foot, ankle, and related structures. These services, including surgery, are generally provided by a licensed doctor of podiatric medicine. Pregnancy and maternity care Benefits are available for maternity care services. Benefits include: • Prenatal care; • Postnatal care; • Involuntary complications of pregnancy; • Inpatient Hospital services including labor, delivery, and postpartum care; • Elective newborn circumcision within 18 months of birth; and • Pregnancy termination services.   • Provision of Professional Services We will provide any Professional Services set out in a relevant Order Form in a professional and workmanlike manner, using reasonable skill and care in accordance with industry practice and any relevant SOW. Where it is necessary for Us or Our Group to visit your sites in order to perform Professional Services you shall at no cost to Us or Our Group: (a) provide reasonable access to any relevant sites at times to be agreed; (b) inform Us of health and safety, and security policies applicable at the sites; (c) ensure the sites are safe;

  • Professional Services Bodily injury" or "property damage" arising out of the rendering of or failure to render profes- sional services;

  • Scope of Professional Services 3.1 On the terms and conditions set forth in this Agreement, COUNTY hereby engages CONTRACTOR to provide all labor, materials and equipment to complete the Project/Service in accordance with the Scope of Services, attached hereto and incorporated herein as Attachment A, as modified or clarified by Addendum(s) # , dated , attached hereto and incorporated herein by reference as Attachment B.   • Professional Services Fees You agree to pay us the professional services fees in the amounts set forth in the Investment Summary. Those amounts are payable in accordance with our Invoicing and Payment Policy. You acknowledge that the fees stated in the Investment Summary are good-faith estimates of the amount of time and materials required for your implementation. We will bill you the actual fees incurred based on the in-scope services provided to you. Any discrepancies in the total values set forth in the Investment Summary will be resolved by multiplying the applicable hourly rate by the quoted hours.
