Physician Certificate. A pregnant employee requesting a leave of absence without pay must present at the time the request is made, a physician’s certificate stating the probable period for which the employee will not be able to perform substantial, material duties of the employee’s position due to pregnancy, childbirth or related medical conditions.
Physician Certificate. The school district may require an employee to furnish a medical certificate from a qualified physician as evidence of illness, indicating such absence was due to illness in order to qualify for sick leave pay.
Physician Certificate. In cases of illness/injury, the employee will provide the administration with certification of illness from the attending physician on the fifth (5) consecutive day of absence. In the event the illness extends beyond ten (10) consecutive school days, the employee will provide the administration with certification of illness, as required.
