PLAN AVAILABILITY. 21.13.1 Upon initial employment of contract and regular employees and upon ratification of the agreement, the District will notify each covered employee of his/her rights under the Consolidated Omnibus Budget Reconciliation Act of 1985 (COBRA) and under Education Code §7000, et seq. "Health and Welfare Benefits."
PLAN AVAILABILITY. Provider agrees to, within three (3) business days of receiving a request, make available to Plan or, at Plan’s request, to Plan Sponsor, PHI that is relevant for Plan to carry out health plan functions under 45 C.F.R. § 164.504(f), provided the disclosures are subject to, and consistent with, the terms of the Provider Agreement.

PLAN AVAILABILITY. Broker shall inform Group(s) of Plan’s availability and shall present Plan’s information materials and full and fair disclosure documents to Group(s). Plan shall provide such materials to Broker as having been approved by the California Department of Managed Health Care pursuant to the Xxxx-Xxxxx Health Care Service Plan Act of 1975, as amended.
PLAN AVAILABILITY. Broker shall market Plan’s products to prospective Clients and shall present Plan’s information materials and full and fair disclosure documents to Client. Plan shall provide such materials and documents to Broker after they have been approved by the DMHC. When permitted, Broker’s duties shall include distribution of the Plan enrollment packet to all individuals who are eligible to enroll upon initial qualification for enrollment and during open enrollment programs
PLAN AVAILABILITY. The permittee shall make the SWPPP and specific inspection reports available upon request to the Administrator or his representative; any federal, state or local government officials or to the operator of a municipal separate storm sewer system receiving discharges from the site.
