Premium Without Wellness Discount. The following payroll deductions set in Table 1 below shall apply unless the teacher and/or spouse qualify for a wellness premium discount: 2016 $50 month/ annual $600 2017 $50 month/ annual $600 $50 month/ annual $600 $100 month/ $1200 annual 2018 $100 month/ annual $1200 $100 month/ annual $1200 $200 month/ $2400 annual * These premiums do not apply to dependent children. (1) The premium without wellness discount for the teacher set forth in Table 1 above shall not apply if the teacher participates in wellness activities that equal 100 points chosen by the teacher from a list provided by Employee Benefits & Insurance Management and the teacher provides proof of such participation to Employee Benefits & Insurance Management. (2) The premium without wellness discount for the teacher’s spouse set forth in Table 1 above shall not apply if the spouse participates in wellness activities that equal 100 points chosen by the spouse from a list provided by Employ- ee Benefits & Insurance Management, and provides proof of such participa- tion to Employee Benefits & Insurance Management. (3) Wellness points earned between September 1 and August 31 apply toward the wellness premium discount for the following plan year except as out- lined below: a. For the 2020 plan year, wellness points completed between September 1, 2018 and August 31, 2019 will apply toward the wellness premium discount for the 2020 plan year. b. Starting in the 2020 plan year, proof of wellness points completed Sep- tember 1 through August 31 will not be accepted or applied toward the premium discount after the close of benefits enrollment. c. For the 2021 plan year only, employees and their spouses are not re- quired to complete wellness points between September 1, 2019 and August 31, 2020, and the payroll deductions required for premium without wellness discount for failure to earn 100 wellness points will not be in effect. d. For the 2022 plan year, wellness points completed between September 1, 2020 and August 31, 2021, will be applied toward the wellness pre- mium discount, and the payroll deductions required for premium with- out wellness discount for failure to earn 100 wellness points will be in effect.

(1) The 2024 wellness premium discount ($1200 per year for an employee and $1200 per year for a spouse) will be applied for the employee (and spouse if applicable) if the employee completes the enrollment process and certifies that one of the following two conditions has been met: a. 100 points of wellness activities have been met from the wellness matrix provided by Employee Benefits & Insurance Management, or b. both an annual physical exam with blood work AND an eye exam have been performed. (2) Services must have been rendered between September 1, 2022 and August 31, 2023 to apply toward the 2024 discount. (3) The employee must complete the enrollment process to receive the wellness discount. If the employee fails to complete the enrollment process, no wellness discount will be provided. (4) To receive the 2025 wellness discount ($1200 per year for an employee and $1200 per year for a spouse), the employee (and spouse if applicable) must have received either an annual physical exam with blood work OR an eye exam between September 1, 2023 and August 31, 2024. (5) Newly hired employees and employees transferring into benefitted positions will automatically receive the wellness discount for the rest of that calendar year. Employees hired after April each year and employees transferring into benefitted positions after April will automatically receive the wellness discount for the rest of that calendar year and the following calendar year.

  • PAYMENT TERMS/DISCOUNTS The County’s payment terms are in accordance with Florida Statute 218, Local Government Prompt Payment Act. Cash discounts for prompt payment shall not be considered in determining the lowest net cost for bid evaluation purposes.

  • Reallocation to a Class with a Lower Salary Range Maximum 1. If the employee meets the skills and abilities requirements of the position and chooses to remain in the reallocated position, the employee retains the existing appointment status and has the right to be placed on the Employer’s internal layoff list for the classification occupied prior to the reallocation. 2.   • Average Log Length and Payment Reduction If the average log length for all logs delivered under this contract is less than the average log length specified in the table in clause G-024.2, The amount of allowable payment reduction shall be calculated by multiplying the payment rate in P-028.2 by the total volume delivered, and the difference between the average length of logs delivered and the average log length specified in G-024.2, times 1% as follows: Log Length Payment Reduction = (B x V x L) x (.01) Where: B = Bid rate from P-028.2 clause V = total delivered log Volume L = Length in feet below specified average (rounded to nearest Average log length payment reductions calculated by the Purchaser must be approved by the State, prior to payment for the final billing period. Third-party scaling organization information is required to determine Xxxxxxxx mbf and Average log length for payment reduction purposes. Average log length is determined on a piece count basis. Value of log length price reduction will be derived from the applicable sort value as described in this contract. Scale information for determining Average log length for payment reduction eligibility must be obtained from roll-out scale. Truck-ramp, sample scaling, and/or bundle scaling information is not acceptable for determining eligibility. Purchaser’s exclusive remedy for below average log lengths shall be the payment reduction described in this clause, notwithstanding other provisions in the Uniform Commercial Code.
