Attendance for the month of service must be entered into the BOARD provided data management system no later than day 5 of the month following the month of service. All records of attendance shall be maintained on the DISTRICT’S SES FORMS.
Records of Attendance. For each invoice, Provider shall submit: a. Monthly records of student attendance on a form or forms provided by the District, including the name, and school of student; b. The hourly rate for the service given to the student; c. The name of provider’s employee who rendered the service; d. The amount of time of such service for each day, measured to the nearest thirty (30) minutes; e. The total number of hours of such service for the month; f. The amount due; and payment will be made only if students received the services. Each record shall be signed by a representative of the Provider and a representative of the District.
Records of Attendance. Provider will keep current all attendance and may be accessed by the district at any time.

  • Court Attendance Any employee covered by this Agreement who may be required to attend any commission, court or hearing, to give evidence in any case, civil or criminal respecting the hotel in which they are employed, shall be compensated at the same hourly rate as called for in this Agreement, with a minimum of four (4) hours pay.

  • PAYMENT FOR MEETING ATTENDANCE 18.01 Where the Employer requires an employee to be present at a meeting scheduled by the Employer, time spent at such meeting shall be considered time worked, in accordance with Article 14.03

  • Perfect Attendance A bonus for perfect attendance will be paid to both full and part-time employees. The amount of $100 will be paid for the first quarter of perfect attendance, $125 for the second accumulated quarter of perfect attendance, $150 for the third accumulated quarter of perfect attendance, and $250 for the fourth quarter of accumulated perfect attendance. Payment for perfect attendance shall be reflected in the second paycheck each January and July for qualifying Association members. These pay days may be adjusted if the school calendar changes. In order to be considered for perfect attendance no sick leave, personal leave days, partial days, or days without pay may be used during the quarter.

  • Records to be kept 8.1 The Recipient must: (a) maintain and operate effective monitoring and financial management systems; and (b) keep a record of expenditure funded partly or wholly by the Grant, and retain all accounting records relating to this for a period of at least six years after the end of the Funding Period. Accounting records include: original invoices, receipts, minutes from meetings, accounts, deeds, and any other relevant documentation, whether in writing or electronic form. 8.2 Where the Recipient is working in partnership and its partner(s) wish to retain such documentation, the Recipient should obtain from the partner(s): (a) an annual, written statement, signed by the partner’s Chief Financial Officer, of how the money was spent; and (b) a signed undertaking that the partner will retain such documents for the period prescribed above. 8.3 The funds provided under this Grant Agreement may not be used to purchase capital items.

  • Conference Attendance The Federation will be entitled to five (5) person days per year release time for designated Federation representatives to attend conferences. All conference expenses will be the responsibility of the Federation, unless funding is requested and approved through regular District processes for conference attendance.

  • Keeping of Records and Books of Account The Company shall keep and cause each subsidiary to keep adequate records and books of account, in which complete entries will be made in accordance with GAAP consistently applied, reflecting all financial transactions of the Company and its subsidiaries, and in which, for each fiscal year, all proper reserves for depreciation, depletion, obsolescence, amortization, taxes, bad debts and other purposes in connection with its business shall be made.

  • Meeting Attendance The Contractor shall attend such meetings of the Town relative to the Scope of Work set forth in Exhibit A as may be requested by the Town.
