REGULAR PERMANENT CLASSIFIED EMPLOYEES. Employees will receive a formal, written evaluation annually. • Each employee will receive a copy of the evaluation form at least one week before the evaluation meeting so the employee can come prepared. For evaluation of instructional assistants only, the teacher input sheet will be distributed two weeks prior to the evaluation meeting. (The teacher input sheet does not go into the personnel file.) • Each employee will receive a copy of her/his evaluation and be allowed to attach a written response. The signature of the employee and that of the immediate supervisor shall be entered on the evaluation form. Should the employee refuse to sign, that fact should be noted on all copies of the evaluation form.

  • Disabled Employees If an employee becomes disabled with the result that he is unable to carry out the regular functions of his position, the Hospital may establish a special classification and salary with the hope of providing an opportunity of continued employment.
