Reimbursement for Attested Time. All Settlement Class Members may submit a claim for reimbursement of Attested Time up to ten (10) hours at forty dollars ($40) per hour. Settlement Class Members can receive reimbursement of Attested Time with a brief description of the actions taken in response to the Data Security Incident and the time associated with each action. Claims for Attested Time are capped at $400 per individual.

A legal guardian for a Settlement Class Member who is under the age of eighteen (18) at the time of claim submission may submit a Minor Claim Form seeking reimbursement of Attested Time on the minor’s behalf.
Reimbursement for Attested Time. Settlement Class Members may submit a claim for up to eight (8) hours of time spent remedying issues related to identity theft directly caused by the Data Breach at $20 an hour by submitting the Claim Form (Other Benefits) by the Claim Deadline (Other Benefits).
Reimbursement for Attested Time. Class Members who submit a claim for Out- of-Pocket Losses may also submit a claim for up to four (4) hours of time spent remedying issues related to identity theft directly caused by the Data Security Incident at $20 an hour by submitting the Claim Form by the Claim Deadline.
Reimbursement for Attested Time. If you submit a claim for Out-of-Pocket Losses and spent time remedying issues related to identify theft directly caused by the Data Security Incident, you can recover up to four (4) total hours at $20 per hour for a maximum total of $80.
Reimbursement for Attested Time. If you submit a claim for Out-of-Pocket Losses, time you had to spend dealing with the effects of the Data Security Incident. You may check more than one box and may check all three if applicable. Please be sure to fill in the total amount you are claiming for each category and attach documentation of the charges described in bold type. If you are asked to provide account statements as part of proof required for any part of your claim, you may mark out any unrelated transactions if you wish.
Reimbursement for Attested Time. If you submit a claim for Reimbursement of Out-of-Pocket Losses and spent time remedying issues related to identify theft directly caused by the Data Security Incident, you can submit a claim for $20 per hour for up to four (4) total hours. If you submit a claim for Reimbursement for Attested Time, you must provide the Settlement Administrator with your contact information as well as the actions you took in response to the Data Breach and the time each action took. You can, but do not need to, submit a claim for Credit Monitoring in order to submit a claim for Reimbursement of Attested Time. A claim for Reimbursement for Attested Time must be combined with a claim for Reimbursement of Out-of-Pocket Losses, but the maximum total amount of both of those combined claims is $150.00. To be eligible to receive Reimbursement for Attested Time, you must submit a completed Claim Form by [90 days after Notice Deadline].
Reimbursement for Attested Time. All Settlement Class Members may submit a claim for reimbursement of Attested Time up to four (4) hours at $20.00 per hour for self-certified Attested Time that is fairly traceable to the Security Incident. Settlement Class Members may submit an additional Attested Time request for up to eight (8) additional hours at $20.00 per hour for documented time that is fairly traceable to the Security Incident and supported by Reasonable Documentation.
Reimbursement for Attested Time. All Settlement Class Members may submit a claim for reimbursement of Attested Time up to three (3) hours at Twenty-Five Dollars and Zero Cents ($25.00) per hour. Settlement Class Members can receive reimbursement of Attested Time with a brief description of the actions taken in response to the Data Breach and the time associated with each action. Settlement Class Members can only receive reimbursement of Attested Time if at least one (1) full hour was spent. Claims for Attested Time are capped at Seventy Five Dollars and Zero Cents ($75.00) per individual. A claim for Attested Time may be combined with a claim for reimbursement for Out-of-Pocket Losses but in no circumstance will a Settlement Class Member be eligible to receive more than the Five Thousand Dollars and Zero Cents ($5,000.00) individual cap for reimbursements of Out-of-Pocket Losses and Attested Time.
