Reimbursement for Attested Time. All Settlement Class Members may submit a claim for reimbursement of Attested Time up to ten (10) hours at forty dollars ($40) per hour. Settlement Class Members can receive reimbursement of Attested Time with a brief description of the actions taken in response to the Data Security Incident and the time associated with each action. Claims for Attested Time are capped at $400 per individual.
Reimbursement for Attested Time. All Settlement Class Members may submit a claim for reimbursement of Attested Time up to twenty (20) hours at twenty-five dollars ($25) per hour. Settlement Class Members can receive reimbursement for Attested Time with a brief description of the actions taken in response to the Data Breach and the time associated with each action. Claims for Attested Time are capped at $500.00 per individual. A claim for Attested Time may be combined with reimbursement for Out-of-Pocket Costs but in no circumstance will a Settlement Class Member be eligible to receive more than the $15,000.00 individual cap.
Reimbursement for Attested Time. All Settlement Class Members may submit a claim for reimbursement of Attested Time up to five (5) hours at twenty-five dollars ($25) per hour. Settlement Class Members can receive reimbursement of Attested Time with a brief description of the actions taken in response to the Data Security Incident and the time associated with each action. Claims for Attested Time are capped at $125.00 per individual. A claim for Attested Time may be combined with reimbursement for Out-of-Pocket Losses but in no circumstance will a Settlement Class Member be eligible to receive more than the $25,000.00 individual cap. A legal guardian for a Settlement Class Member who is under the age of eighteen (18) at the time of claim submission may submit a Minor Claim Form seeking reimbursement of Attested Time on the minor’s behalf.
Reimbursement for Attested Time. All Settlement Class Members may submit a claim for reimbursement of Attested Time up to five (5) hours at Twenty Dollars and Zero Cents ($20.00) per hour. Settlement Class Members can receive reimbursement of Attested Time with a brief description of the actions taken in response to the Data Breach and the time associated with each action. Claims for Attested Time are capped at One Hundred Dollars and Zero Cents ($100.00) per individual. A claim for Attested Time may be combined with a claim for reimbursement for Ordinary Out-of-Pocket Losses but in no circumstance will a Settlement Class Member be eligible to receive more than the Seven Hundred Fifty Dollars and Zero Cents ($750.00) individual cap.
Reimbursement for Attested Time. Settlement Class Members may submit a claim for up to eight (8) hours of time spent remedying issues related to identity theft directly caused by the Data Breach at $20 an hour by submitting the Claim Form (Other Benefits) by the Claim Deadline (Other Benefits).
(i) To receive Reimbursement for Attested Time, Settlement Class Members must provide to the Settlement Administrator information required to evaluate the claim, including: (1) the Class Member’s name and current address; (2) a brief description of the time incurred including activities undertaken by the Class Member; and (3) an attestation that the information is being provided under penalty of perjury.
Reimbursement for Attested Time. All Settlement Class Members may submit a claim for reimbursement of Attested Time up to four (4) hours at Twenty Five Dollars and Zero Cents ($25.00) per hour, but only if at least one (1) full hour was spent. Settlement Class Members can receive reimbursement of Attested Time with a brief description of the actions taken in response to the Data Incident and the time associated with each action. Claims for Attested Time are capped at One Hundred Dollars and Zero Cents ($100.00) per individual. A claim for Attested Time may be combined with a claim for reimbursement for Ordinary Out-of-Pocket Losses but in no circumstance will a Settlement Class Member be eligible to receive more than the Five Hundred Dollars and Zero Cents ($500.00) individual cap.
Reimbursement for Attested Time. Settlement Class Members may submit a claim for up to three (3) hours of time spent remedying issues related to identity theft directly caused by the Data Incident (“Attested Time”) at Twenty Dollars and Zero Cents ($20.00) per hour by submitting the Claim Form by the Claim Deadline.
(i) Settlement Class Members who elect to submit a claim for Reimbursement for Attested Time must provide to the Settlement Administrator the information required to evaluate the claim, including a brief description of the time incurred including the activities undertaken by the Settlement Class Member.
(ii) The Settlement Administrator shall have the sole discretion and authority to determine whether the prerequisites have been met in order to award payments for Attested Time. The Settlement Administrator may contact Settlement Class Members to seek clarification regarding any submitted claim prior to determining their validity.
(iii) Claims for Reimbursement for Attested Time can be combined with claims for Reimbursement for Ordinary Out-of-Pocket Losses; however, those claims are subject to a combined $500.00 aggregate cap. In no event shall an Approved Claim for Reimbursement of Ordinary Out-of-Pocket Losses and Reimbursement for Attested Time exceed, in the aggregate, Five Hundred Dollars and Zero Cents ($500.00).
Reimbursement for Attested Time. Class Members who submit a claim for Out- of-Pocket Losses may also submit a claim for up to four (4) hours of time spent remedying issues related to identity theft directly caused by the Data Security Incident at $20 an hour by submitting the Claim Form by the Claim Deadline.
(i) Class Members who elect to submit a claim for reimbursement for attested time must provide to the Settlement Administrator the information required to evaluate the claim, including: (i) the Class Member’s name and current address; and (ii) a brief description of the time incurred including the activities undertaken by the Class Member.
(ii) The Settlement Administrator shall have the sole discretion and authority to determine whether the prerequisites have been met in order to award payments for attested time. The Settlement Administrator may contact Class Members to seek clarification regarding any submitted claim prior to determining their validity.
(iii) Claims for Reimbursement for Out-of-Pocket Losses and Reimbursement for Attested Time are subject to a combined $150.00 aggregate cap per Class Member.
Reimbursement for Attested Time. If you submit a claim for Out-of-Pocket Losses and spent time remedying issues related to identify theft directly caused by the Data Security Incident, you can recover up to four (4) total hours at $20 per hour for a maximum total of $80.
Reimbursement for Attested Time. If you submit a claim for Out-of-Pocket Losses, time you had to spend dealing with the effects of the Data Security Incident.
A. CREDIT MONITORING (Check box if you want this benefit) The Settlement requires St. Xxxxxxx to provide three years of credit monitoring and identity protection services through Pango Identity Defense Complete to any class member who timely claims it. The three years of service is above and beyond any services previously offered by St. Xxxxxxx following the Data Security Incident. Pango Identity Defense Complete includes credit monitoring from one bureau, dark web monitoring, monthly credit scores, access to credit reports, authentication alerts, high risk transaction monitoring, address change monitoring, and $1 million in identity theft.
