Requirement to Provide. Certain Information in Public Communications‌ Pursuant to P.L. 114-113, Division H, Title V, Section 505, when issuing statements, press releases, requests for proposals, bid solicitations and other documents describing projects or programs funded in whole or in part with Federal money, all non-Federal entities receiving Federal funds shall clearly state: 1. The percentage of the total costs of the program or project which will be financed with Federal money; 2. The dollar amount of Federal funds for the project or program; and 3. The percentage and dollar amount of the total costs of the project or program that will be financed by non-governmental sources. The requirements of this part are separate from those in the 2 CFR 200 and, when appropriate, both must be complied with.

Requirement to Provide information to the Principal (a) On request of the Principal or the Principal’s Representative, the Supplier must promptly provide to the Principal, at no cost, copies of any Agreements entered into with any Eligible Customers and any other information reasonably requested by the Principal. (b) Notwithstanding any other provision of this SOA or any Agreement, the Supplier acknowledges and agrees that the Principal and Eligible Customers may make available information and materials concerning or relating to the Supplier or its performance under this SOA and any Agreement to each other and/or to any Government Agency.
Requirement to Provide a Physical Inventory Thirty (30) days prior to the end of this Sub-grant Agreement, Sub- grantee shall present CSBD with a physical inventory of all property located in or out of the CSBD, One-Stop Centers purchased with CSBD funds by Sub-grantee or CSBD. The physical inventory shall contain a reference to the Sub-grant Agreement under which funds were allocated for the purchase along with the CSBD inventory tag number.

Requirement to Provide supporting evidence‌ Unless otherwise agreed by the Registrar, an employee will be required to provide medical certificates or statutory declarations to explain any period of personal leave in the following circumstances: • absence for 3 consecutive days or more • absence on the working days either side of a public holiday (e.g. Public Holiday Monday 27 – employee absent Friday 24 and Tuesday 28) • where there is an unexplained pattern of absence • where the employee has accessed a total of 10 days personal leave without a medical certificate or statutory declaration in a calendar year. Not withstanding the above, the Registrar may request that an employee provide a medical certificate or statutory declaration for any period of personal leave. Medical certificates may be accepted from but are not limited to: • medical and dental practitioners • accredited health practitioners and therapists • traditionally or culturally appropriate healers.
