SERVICE REPORTING REQUIREMENTS Sample Clauses

SERVICE REPORTING REQUIREMENTSOn request, the Provider shall supply copies of documents from the State of Illinois reflecting services that have been presented to the State of Illinois for payment, fee for service contracts or grant funding agreements held with any State Department, with all applicable exhibits or attachments, when funded services fall under 59 Illinois Administrative Code, Part 115, Part 119, Part 120, Part 132, Part 140, and 77 Illinois Administrative Code Part 2060 and Part 2090 in order to coordinate benefits and Network service capacity. The Provider shall provide reports in either paper or electronic format acceptable to the Mental Health Board. Mental Health Board billing affidavits shall be accompanied by the submission of data as specified in the approved Mental Health Board Work Plan to substantiate the payment request, as well as all other financial and billing documentation reasonably requested by the Mental Health Board. Failure of Provider to submit the requested information to the Mental Health Board will be considered a breach of this Agreement. All information submitted by the Provider shall comply with the confidentiality requirements of state and federal law.
SERVICE REPORTING REQUIREMENTSThe Provider shall submit monthly service reports indicating the actual units of service provided. These reports are due to DDSN as outlined in the DDSN Finance Manual.
SERVICE REPORTING REQUIREMENTS. On request, the Provider shall provide copies of documents from the Department of Human Services reflecting clinical services that have been presented to the Department of Human Services for payment when such services fall under 59 Illinois Administrative Code, Part 132 in order to coordinate billing and service capacity. The Provider shall provide reports in either paper or electronic format acceptable to the Mental Health Board for Service planning purposes. Billing affidavits shall be accompanied by the electronic submission of Service lines and shall include demographic and Service data as specified in the approved Work Plan to substantiate the payment request, as well as all other financial and billing documentation reasonably requested by the Mental Health Board. Failure of Provider to submit the information requested to the Mental Health Board in a timely manner will be considered a breach of this Agreement. All information submitted by the Provider shall comply with the confidentiality requirements of state and federal law.
SERVICE REPORTING REQUIREMENTSPlanning meeting Within ninety (90) calendar days of execution of Amendment No. 1 to this Contract, Contractor will meet with DSHS TCR staff to discuss and finalize dates for live class training utilizing a webinar platform and a live question and answer session. The Parties will also discuss course curricula development and finalize a timeline for deliverables. Completion deadline: Within ninety (90) calendar days of execution of Amendment No. 1 to this Contract. Curricula development Contractor will develop the following: curricula, curricula outline, syllabi, handouts, class evaluation surveys, and a webinar. Course curricula will train both TCR staff and Texas Cancer Reporters. Classes will be conducted live by webinar. The webinar platform will record the classes and host up to two hundred and fifty (250) trainees at once. Contractor will provide training for ten (10) classes, including five (5) separate training classes for TCR staff, and five (5) separate training classes for Texas Cancer Reporters. Classes must provide practical, hands-on exercises and activities. Completion deadline: January 31, 2024 Curricula for all five (5) training classes approved by DSHS TCR staff Contractor will submit the following for approval to TCR staff no later than thirty (30) calendar days before the first training class: course curricula, curricula outline, syllabi, handouts, class evaluation surveys, and trainings for all five (5) classes. Completion deadline: January 31, 2024 Information required by NCRA for all classes will be submitted to NCRA for approval and continuing education (“CE”) credit By January 31, 2024, Contractor will submit the following for approval to NCRA prior to the first TCR staff live training class: (1) all information required for NCRA approval of training course curricula; and (2) all information required to receive CE credit. Completion deadline: January 31, 2024 First class conducted for TCR staff Contractor will conduct the first class for TCR staff. Completion deadline: August 31, 2024 Incorporate revisions to course material following first TCR staff class, if necessary TCR staff will provide recommendations to Contractor regarding updating the course material used in the first TCR staff class. After receipt of these recommendations from TCR staff, Contractor will incorporate recommended changes to the material from the first TCR staff class prior to conducting the initial Texas Cancer Reporters class. Completion deadline: August 31...
SERVICE REPORTING REQUIREMENTS. Jurisdictional factors shall be developed and reported for particular services as specified in the BellSouth Tariffs and as specified in applicable contracts that are provisioned for a carrier. Factor reporting requirements for these services are discussed in the following subsections.

Failure of Provider to submit requested information to the WCCMHB shall be considered a breach of this Agreement. All information submitted by the Provider shall comply with the confidentiality requirements of state and federal law.

  • Child Abuse Reporting Requirements A. Grantees shall comply with child abuse and neglect reporting requirements in Texas Family Code Chapter 261. This section is in addition to and does not supersede any other legal obligation of the Grantee to report child abuse. B. Grantee shall use the Texas Abuse Hotline Website located at xxxxx://xxx.xxxxxxxxxxxxxx.xxx/Login/Default.aspx as required by the System Agency. Grantee shall retain reporting documentation on site and make it available for inspection by the System Agency.

  • Forecasting Requirements 19.5.1 The Parties shall exchange technical descriptions and forecasts of their Interconnection and traffic requirements in sufficient detail necessary to establish the Interconnections necessary for traffic completion to and from all Customers in their respective designated service areas.

  • Additional Reporting Requirements Contractor agrees to submit written quarterly reports to H-GAC detailing all transactions during the previous three (3) month period. Reports must include, but are not limited, to the following information: a. Customer Name b. Product/Service purchased, including Product Code if applicable c. Customer Purchase Order Number

  • Testing Requirements 12.1. Workplaces - 12.2. On workplaces where the value of the Commonwealth’s contribution to the project that includes the building work is at least $5,000,000, and represents at least 50% of the total construction project value or the Commonwealth’s contribution to the project that includes the building work is at least $10,000,000 (irrespective of its proportion of the total construction project value) the following minimum testing requirements must be adhered to.

  • Reporting Requirement (1) In the event the Contractor identifies covered telecommunications equipment or services used as a substantial or essential component of any system, or as critical technology as part of any system, during contract performance, or the Contractor is notified of such by a subcontractor at any tier or by any other source, the Contractor shall report the information in paragraph (d)(2) of this clause to the Contracting Officer, unless elsewhere in this contract are established procedures for reporting the information; in the case of the Department of Defense, the Contractor shall report to the website at xxxxx://xxxxxx.xxx.xxx. (ii) Within 10 business days of submitting the information in paragraph (d)(2)(i) of this clause: any further available information about mitigation actions undertaken or recommended. In addition, the Contractor shall describe the efforts it undertook to prevent use or submission of covered telecommunications equipment or services, and any additional efforts that will be incorporated to prevent future use or submission of covered telecommunications equipment or services.

  • General Reporting Requirements The MA-PD Sponsor agrees to submit to information to CMS according to 42 CFR §§423.505(f), 423.514, and the “Final Medicare Part D Reporting Requirements,” a document issued by CMS and subject to modification each program year.
