Solicitation of Donations. Annually, the District shall solicit donations to the Catastrophic Illness or Injury Leave Bank. The annual solicitation shall occur in April. More frequent solicitations by the District may be made if the Catastrophic Illness or Injury Committee deems it appropriate.
Solicitation of Donations. Contributions to the Catastrophic Leave Bank shall be solicited during the month of November each school year if there are fewer than 1,280 hours remaining in the Bank. All donation forms must be received by the Payroll Office no later than the last working day in December of each school ear. In addition, donations may be solicited when needed to replenish the Catastrophic Leave Bank.

Contributions for the Sick Leave Bank shall be donated by the last work day in October of each year. At the beginning of the donation period, the Human Resources Office shall send to each unit member a notice outlining the Sick Leave Bank program and a “Sick Leave Bank Form.” All donations must be received by the Human Resources Office by the last work day in October.

Any unit member newly hired shall be given the opportunity to 930 join the CSLB within thirty (30) days of hire. If the CSLB balance falls 931 below thirty (30), the CSLB Committee shall be notified. The 932 committee may request an additional thirty (30) day donation period. 933 C. CSLB will be capped at 100 days. Once capped, members who have 934 already donated from the previous year will be grandfathered in without 935 providing a donation for the year the cap is met. In the year when the 936 cap is met, members who have not donated the previous year may 937 donate one day during the new enrollment period. 938 D. Catastrophic Sick Leave Bank Approval Committee: All unit members 939 wishing to use the CSLB shall submit a “Catastrophic Leave Request 940 Form.” This form shall be submitted to the personnel office. The 941 request shall state the estimated number of days being requested by 942 the unit member. The CSLB Committee shall consider the request of 943 the unit member within fifteen (15) working days of submission of the 944 request. The committee shall consist of the Superintendent, the WTA 945 President and the WTA Lead Negotiator. Designees may be appointed 946 by mutual agreement.
