Special Features of PACE CNY. PACE CNY, a Program of All-inclusive Care for the Elderly, is a special program authorized by New York State and the Centers for Medicare and Medicaid Services (CMS) designed to provide an alternative to those elders who prefer not to move into a nursing home, but whose medical problems make it impossible for them to stay at home without the help from doctors, nurses, social workers, and other caregivers. If you decide to join PACE CNY, you will be enrolling in a managed long-term care plan. Managed Care is a way to provide you with all the health and long term care services you need from one responsible program, such as PACE CNY. Our program will coordinate all your health and long term care needs through its doctors, nurses, personal care attendants, therapists and other members of the PACE CNY interdisciplinary team. We will provide you with a list of all providers in the PACE CNY network. When selecting PACE CNY, you agree to only receive services from the providers on that list. By managing all your health and long term care services through an interdisciplinary team, problems are identified, preventive care promoted and hopefully emergency room or hospital stays avoided. PACE CNY strives to keep seniors as active as possible and living at home as long as is feasible. Special features of the program include:

  • Alternate Traffic Routing If CLEC has a LIS arrangement which provides two (2) paths to a CenturyLink End Office Switch (one (1) route via a Tandem Switch and one (1) direct route), CLEC may elect to utilize alternate traffic routing. CLEC traffic will be offered first to the direct trunk group (also referred to as the "primary high" route) and then overflow to the Tandem Switch group (also referred to as the "alternate final" route) for completion to CenturyLink End Office Switches.

  • PRICING STRUCTURES Licenses and Support Services for the Licensed Programs to which this OST applies are granted according to the pricing structures mentioned in the related Transaction Document. Standard pricing structures are defined in the section “DEFINITIONS” of this OST, even though those pricing structures may not be applicable to the DS Offerings to which this OST applies. Other pricing structures may be made available on a case-by-case basis.

  • SPECIAL TOOLS & TEST EQUIPMENT If the price stated on the Offer includes the cost of any special tooling or special test equipment fabricated or required by the Contractor for the purpose of filling this order, such special tooling equipment and any process sheets related thereto shall become the property of the City and shall be identified by the Contractor as such.

  • Alternative Work Schedule An alternate forty (40) hour work schedule (other than five (5) uniform and consecutive eight (8) hour days in a seven (7) day period), or for hospital personnel an eighty (80) hour workweek in a fourteen (14) day period and other mutually agreed upon schedules that comply with applicable federal and state law. Employee work schedules normally include two (2) consecutive days off.

  • Special Access Service Conversions 5.3.6.1 IDS may not convert special access services to combinations of loop and transport network elements, whether or not IDS self-provides its entrance facilities (or obtains entrance facilities from a third party), unless IDS uses the combination to provide a significant amount of local exchange service, in addition to exchange access service, to a particular customer. To the extent IDS requests to convert any special access services to combinations of loop and transport network elements at UNE prices, IDS shall provide to BellSouth a letter certifying that IDS is providing a significant amount of local exchange service (as described in this Section) over such combinations. The certification letter shall also indicate under what local usage option IDS seeks to qualify for conversion of special access circuits. IDS shall be deemed to be providing a significant amount of local exchange service over such combinations if one of the following options is met:
