Staffing and Reporting Requirements. 1.16.1. As requested by State, Contractor shall participate in meetings with State staff, providers and other State or legislative officials. State may request additional employees from Contractor to participate as needed. 1.16.2. Contractor shall be responsible for preparing reports and documents, as well as providing analytic support and reimbursement subject matter expertise on projects, as requested by State. 1.16.3. Contractor shall participate in regularly scheduled check-in calls with State program staff as requested and ad hoc meetings with State staff, providers and other State or legislative officials to support the work of the VMNG program and Medicaid value-based initiatives. 1.16.4. Contractor shall participate in check-in calls with State program staff as requested by State to support the Rate Setting work under the Contract. The frequency of these calls may vary depending on the needs of State. The maximum dollar amount payable under this contract is not intended as any form of a guaranteed amount. The Contractor will be paid for products or services actually delivered or performed, as specified in Attachment A, up to the maximum allowable amount specified on page 1 of this contract. 1. Prior to commencement of work and release of any payments, Contractor shall submit to the State: 1.1. a certificate of insurance consistent with the requirements set forth in Attachment C, Section 8 (Insurance), and with any additional requirements for insurance as may be set forth elsewhere in this contract; and 1.2. a current IRS Form W-9 (signed within the last six months).

The Grantee or Management Company shall complete a CRF Subsidy Request Report - Recap of Tenant Income Certification, which provides a unit-by-unit listing of all units in the Development for whom assistance is being requested and gives detailed information including the occupants’ eligibility, set-aside requirements, amount of household rent paid, utility allowance and amount of CRF Rental Subsidy requested. 2. The CRF Subsidy Request Report - Recap of Tenant Income Certification shall be prepared as of the last day of each calendar month during the period of performance and shall be submitted to XXXXxxxxxxxx@XxxxxxxXxxxxxx.xxx and Florida Housing’s monitoring agent no later than the 15th day of the following month. The December 2020 request will be due on or before December 15th. The Grantee will submit executed Coronavirus Relief Fund Rental Assistance Applications and supporting documentation to Florida Housing’s monitoring agent within 5 days upon the monitoring agent’s request.

  • Monitoring and Reporting The Programme Operator shall monitor, record and report on progress towards the programme’s outcomes in accordance with the provisions contained in the legal framework.
