Common use of Student Medication Clause in Contracts

Student Medication. 1. Except in emergency situations, teachers will only be required to administer medication to students (including supervision of self-administration) after the following conditions have been met: a. The medication is required while the student is attending school. b. The student's parent(s) or legal guardian(s) has requested the school's assistance and signed a written release absolving the Board of Education, the school and the teacher(s) in question from any and all liability arising from the administration of the medication. c. Written authorization has been received from the student's attending physician. d. The School District 8 “Request for Medication at School Form”, must be completed for the administration of emergency or ongoing medications. e. The teacher has received adequate training from the Public Health Nurse concerning the administration of the medication.
