Submission of Statement of Account. 4.01 In order to obtain payment for any fees, you must submit to us a written Statement of Account in a form satisfactory to us on total satisfactory completion of the services. a) You may submit an invoice once monthly for all field work completed and submitted. The total amount payable for will be 60% of the total unit value. b) The remainder of payment (40%) will be paid after all submissions and declarations are completed and in the Ministry of Forests, Lands and Natural Resource Operations’ electronic tracking system (RESULTS). 4.02 The Statement of Account(s) must show the following: a) your legal name, address, the date and the period of time which the invoice applies (“Billing Period”), the contract number, and a statement number for identification; b) the calculation of all fees claimed under this Agreement for the Billing Period, with a description of specific services/works completed during the Billing Period, including a declaration that the Services have been completed; c) and where expenses are to be paid under this Agreement, a chronological listing, in reasonable detail and with dates, of all expenses claimed by you under this Agreement for the Billing Period with receipts or copies of receipts, where applicable, attached; d) if you are claiming reimbursement of any GST or other applicable taxes paid or payable by you in relation to those expenses, a description of any credits, rebates, refunds, or remissions you are entitled to from the relevant taxation authorities in relation to those taxes; e) the calculation of any applicable taxes payable by us in relation to the Services provided under this Agreement and for the Billing Period as a separate line item (excluding taxes paid directly by you to a supplier and which were included in the bid price); f) any other billing information reasonably requested by us. 4.03 Within thirty days of our receipt of your invoice, or the date we authorize payment, whichever is the latter, we must pay you fees and, where applicable, expenses for those Services we determined were satisfactorily received during the Billing Period. 4.04 Invoices are to be submitted to:

However, the Contractor may submit a statement of account in respect of Services provided pursuant to more than one ASR, provided the end of the Service Period of any such ASRs is no more than 30 days prior the date on which the Contractor submits such statement of account. 6.4 If one or more Aircraft are, pursuant to an ASR issued by BC Wildfire Services, transferred to another Fire Centre during a Service Period, the Contractor must submit its Statement of Account to the Fire Centre at the location where the Service Period comes to an end. 6.5 If a Contractor is to be paid fees at a unit rate or is to be paid or reimbursed for expenses, the Contractor must maintain time records and books of account, invoices, receipts and vouchers of expenses in support of those payments, in form and content satisfactory to the Requisitioning Ministry. Unless otherwise specified in this Framework Agreement, the Contractor must retain such documents for a period of not less than seven years after the expiration of this Framework Agreement. 6.6 The Contractor will ensure any statement of account shows the following: (a) Contractor’s legal name, address, date of the statement, statement number, and the contract number indicated on the ASR; (b) The calculation of all fees claimed under the ASR for the Billing Period (defined below) including: (i) the period of time which the invoice applies (the “Billing Period”), (ii) Flight Ticket for each day that supports all charges and expenses to be reimbursed by the Province, regardless of whether any hours flown, including: (A) fuel consumption: (I) volumes (including metre tickets with receiver's printed name and signature, if issued) (II) location drawn from and owner of the fuel, and (III) indicating applicable charges to Province (include receipts if from other contractor), or (IV) indicating if fuel was supplied by the Province, (B) number of landings, including aerodrome identifier, (C) legs of flight, including: (I) from/to and up/down times, (II) hours or miles flown, ...

  • Accuracy of Statements Neither this Agreement nor any Schedule, Exhibit, statement, list, document, certificate or other information furnished by or on behalf of the Company to the Purchaser in connection with this Agreement or any of the transactions contemplated hereby contains any untrue statement of a material fact or omits to state a material fact necessary to make the statements contained herein or therein, in light of the circumstances in which they are made, not misleading.

  • Conclusiveness of Statements; Survival of Provisions Determinations and statements of any Lender pursuant to Sections 8.1, 8.2, 8.3 or 8.4 shall be conclusive absent demonstrable error.   • Security of State Information The Contractor represents and warrants that it has implemented and it shall maintain during the term of this Contract the highest industry standard administrative, technical, and physical safeguards and controls consistent with NIST Special Publication 800-53 (version 3 or higher) and Federal Information Processing Standards Publication 200 and designed to (i) ensure the security and confidentiality of State Data; (ii) protect against any anticipated security threats or hazards to the security or integrity of the State Data; and (iii) protect against unauthorized access to or use of State Data. Such measures shall include at a minimum: (1) access controls on information systems, including controls to authenticate and permit access to State Data only to authorized individuals and controls to prevent the Contractor employees from providing State Data to unauthorized individuals who may seek to obtain this information (whether through fraudulent means or otherwise); (2) industry-standard firewall protection; (3) encryption of electronic State Data while in transit from the Contractor networks to external networks; (4) measures to store in a secure fashion all State Data which shall include multiple levels of authentication; (5) dual control procedures, segregation of duties, and pre-employment criminal background checks for employees with responsibilities for or access to State Data; (6) measures to ensure that the State Data shall not be altered or corrupted without the prior written consent of the State; (7) measures to protect against destruction, loss or damage of State Data due to potential environmental hazards, such as fire and water damage; (8) staff training to implement the information security measures; and (9) monitoring of the security of any portions of the Contractor systems that are used in the provision of the services against intrusion on a twenty-four (24) hour a day basis.
