TEMPORARY SHELTER SERVICES. A. Provision of Temporary Shelter Services. CONTRACTOR shall provide temporary shelter services for up to seven (7) eligible homeless families referred by DHHS – Social Services as part of the Betty’s House Program. CONTRACTOR shall notify COUNTY of any openings in its temporary shelter facility in order to attain and maintain full program capacity, which will be determined based on the size and number of families receiving temporary shelter services pursuant to the terms and conditions of this Agreement. CONTRACTOR shall, to the extent possible, accommodate the special dietary requirements of any individual receiving temporary shelter services pursuant to the terms and conditions of this Agreement.

TEMPORARY SHELTER SERVICES. CONTRACTOR shall provide non-reserved temporary shelter services, as available and not to exceed six (6) placements on any given day, to eligible youth who are within the jurisdiction of the juvenile court pursuant to Section 300 or Section 601 of the California Welfare and Institutions Code. The temporary shelter services provided pursuant to the terms and conditions of this Agreement shall include, without limitation, all of the following: 1. Providing eligible youth with personal living quarters in safe and secure facilities which meet any and all applicable building, fire and health safety standards. 2. Providing adequate supervision and care of eligible youth, including, without limitation, providing food, clothing, school supplies, personal incidentals, reasonable travel to a youth's home for visitation and reasonable travel necessary for a youth to remain in the school in which he or she is enrolled at the time of placement. 3. Providing, in collaboration with DHHS - Social Services staff, eligible youth with necessary support services, including, without limitation, crisis counseling, case management and aftercare services and family team meetings to discuss safety, current needs and service, treatment and discharge planning. 4. Providing appropriate DHHS - Social Services staff with monthly census reports which include, without limitation, the name and birth dates, if available, of each youth receiving temporary shelter or long-term transitional housing placement services pursuant to the terms and conditions of this Agreement during the applicable reporting period along with the number of times each youth has stayed at the shelter, including the date of the most recent stay. 5. Providing the DHHS - Children and Family Services Placement Unit Supervisor with copies of any and all incidents reports involving youths receiving temporary shelter services provided pursuant to the terms and conditions of this Agreement.

  • Surgery Services This plan covers surgery services to treat a disease or injury when: • the operation is not experimental or investigational, or cosmetic in nature; • the operation is being performed at the appropriate place of service; and • the physician is licensed to perform the surgery. This plan covers reconstructive surgery and procedures when the services are performed to relieve pain, or to correct or improve bodily function that is impaired as a result of: • a birth defect; • an accidental injury; • a disease; or • a previous covered surgical procedure. Functional indications for surgical correction do not include psychological, psychiatric or emotional reasons. This plan covers the procedures listed below to treat functional impairments. • abdominal wall surgery including panniculectomy (other than an abdominoplasty); • blepharoplasty and ptosis repair; • gastric bypass or gastric banding; • nasal reconstruction and septorhinoplasty; • orthognathic surgery including mandibular and maxillary osteotomy; • reduction mammoplasty; • removal of breast implants; • removal or treatment of proliferative vascular lesions and hemangiomas; • treatment of varicose veins; or • gynecomastia.

  • ELECTRICAL SERVICES The Company must construct and reticulate electrical requirements for all amenities and facilities. The Company must construct sub-station and distribution boards necessary to reticulate power to all Company owned or leased facilities which provide amenities to the public. The electrical installation must be to the design and installation standards of the State Energy Commission of Western Australia. All electrical reticulation must be placed underground.
