Xxxx Leave Documentation Sample Clauses

Xxxx Leave Documentation. The District will maintain absence records for all teachers. An incidence of absence is defined as one (1) day of nonattendance which invokes the use of a sick day or results in an unpaid absence if no sick days are available, except for an approved Family Medical Leave. In cases of absences of three (3) or more consecutive work days due to illness or injury, the District may require a physician’s statement certifying that the teacher’s condition prevented him/her from appearing for work and indicating that the teacher is fit to return to full duty. If the District requires a physician’s statement from a teacher, the District will reimburse the teacher for the teacher’s examination by the teacher’s physician if the physician determines that the teacher’s condition prevented him/her from reporting for work, to the extent that the teacher’s examination is not covered by insurance. In such a circumstance, the District’s obligation to reimburse the teacher will be limited to the cost of examination as well as any accompanying tests or procedures as part of the examination.
