Family Plan definition

Family Plan means a Plan in which the Subscriber is enrolled with one (1) or more Dependents. For additional information on Newborns during the first sixty (60) days from birth and Adopted Children during first sixty (60) days from the date the Subscriber, enrolled spouse, or enrolled Domestic Partner is granted the right to control health care for an Adopted Child, refer to the section titled YOUR ELIGIBILITY.
Family Plan means a written document developed at the OFDM that includes family recommendations on planning for the child and may include a permanency plan, concurrent permanent plan, placement recommendations, or service recommendations. The "family plan" also includes expectations of the parents of the child and other family members; services the Department will provide; timelines for implementation of the plan; benefits of compliance with the plan; consequences of noncompliance with the plan; and a schedule of future meetings if appropriate. The "family plan" described in ORS 417.375(1) is incorporated into the case plan to the extent that it protects the child, builds on family strengths, and is focused on achieving permanency for the child within a reasonable time.
Family Plan means you and/or your legal spouse with any number of your Child travelling together for the entire Trip.

Examples of Family Plan in a sentence

  • If you are enrolled in an Individual Plan when your child is born, the coverage for thirty- one (31) days described above means your plan becomes a Family Plan for as long as your child is covered.

  • Applicable Family Plan deductibles and maximum out-of-pocket expenses may apply.

  • A Contract Holder enrolled on this Individual and Family Plan who subsequently wishes to change coverage for all Members covered under the plan to another Individual and Family Plan may change to any other open Individual and Family Plan, provided that all premium, Out-of-Pocket Expenses and charges incurred for non-Covered Services are paid in full during the annual open enrollment periods or during a special enrollment period as long as they meet a qualifying event.

  • Family Plan OOP MaximumOnce the members of your family have reached the family plan OOP maximum, all members will no longer be responsible for deductibles, co- insurance, or co-payments for covered services for the remainder of the benefit year (even if a member has not met their individual plan OOP maximum).⇒ Please see Section 2 of this chapter for details on your OOP maximum.
