A Personal. Flexibility Agreement may, for example, provide for a personal arrangement as to the hours within which work is to be performed (whether at ordinary or penalty rates); configuration of working hours; and/or an all-inclusive or ‘loaded’ salary (that may have regard to eg. out of ordinary hours; split working day arrangement; personal configuration for hours work; on-call and/or recall; availability and/or work performed out of hours whether at a workplace or by telephone and/or electronic means; annualised salary having regard to working arrangement and/or anticipated out of hours work; the particular nature of the work being performed; or otherwise). Other than in relation to personal details, the content of a Personal Flexibility Agreement will not be confidential.
A Personal. Xxxxx’s Leave year is defined as the twelve (12) month period commencing on the Personal/Xxxxx’s Leave crediting date and finishing the day before the next Personal/Xxxxx’s Leave crediting date.
A Personal. ILLNESS Day shift custodians calling in sick during the school year must notify the Facilities Supervisor no later than one (1) hour prior to the start of their shift. If unable to reach the Facilities Supervisor, the day shift custodian must notify the Facilities Management Secretary at home. Afternoon shift custodians calling in sick during the school year must notify the Facilities Management Office two (2) hours prior to the start of their shift. The Board reserves the right at such time to require a physical or mental examination of a custodian at the Board’s expense by a doctor of its choice should this seem in the best interests of the school district. In lieu of such an examination, the Board may request a statement from the custodian’s physician regarding the need for the use of sick leave. A written statement may be required from the attending physician in cases of injury or illness that keeps a custodian from work for three (3) or more consecutive working days or following any period of hospitalization. A written statement shall be mandatory commencing the eighth (8) calendar day of absence. Also, such a statement shall be mandatory in all cases of absence covered by Workers Compensation and absence requiring hospitalization regardless of the length of absence. Leave time used for personal illness shall be figured in quarter (1/4) day increments.
A Personal. Carer’s Leave year is defined as the 12 month period commencing on the Personal/Xxxxx’s Leave crediting date and finishing the day before the next Personal/Xxxxx’s Leave crediting date.
A Personal. Health Picture tool that serves as a patient’s personal health record to assist in self-management of chronic conditions.
A Personal. Identification Number (hereinafter called “PIN”) shall be issued for the Credit Card unless the Cardmember stipulated otherwise in the Credit Card application form. The Cardmember hereby authorises the Bank to deliver his PIN to him at the Cardmember’s own risk by whatever means the Bank deems fit and the Cardmember agrees to hold the Bank harmless from all claims and liabilities, loss or damage incurred or suffered in the event the PIN is intercepted or retained by any other person or lost in transit. The PIN is strictly confidential and should not be disclosed to any other person under any circumstance or by any means whether voluntarily or otherwise, as the Cardmember is the only person authorised by the Bank to have knowledge of the PIN.
A Personal. Leave Day may be used for any purpose at the discretion of the teacher, except on the day before or after any holiday and/or the day before or after any vacation day and/or any day not scheduled as a full day of student attendance. A teacher planning to use a Personal Leave Day shall notify her/his principal/supervisor at least two days in advance except in cases of emergency.
