If you require medical and other forms to be completed for third parties, please let your provider know at the beginning of your session and your provider will discuss the completion of the forms while they meet with you in your session. Disability and FMLA paperwork: To the extent permitted by law, there will be a charge of $50.00 for the completion of medical forms up to 5 pages. Requests for Disability Psychiatry of Texas does not accept patients seeking treatment for the sole purpose of obtaining disability benefits or patients seeking long-term disability benefits. It is possible that after evaluating you, your provider may be willing to complete disability paperwork on your behalf, however, your provider is not required to do so and Psychiatry of Texas may assess a fee, as permitted by applicable law, to assist with such a request. Your provider may also need you to schedule a separate follow-up appointment with them for this purpose. Requests for Substance Use Disorder Treatment Psychiatry of Texas providers generally do not provide treatment for substance use disorders, such as treatment for detoxification, acute withdrawal, or medication-assisted treatment in an outpatient setting. Consequently, if necessary, you may be required to be admitted to inpatient treatment at a hospital. If you require substance use disorder treatment, please reach out to your insurance plan for assistance in finding an appropriate treatment provider or facility.
