Accident Investigation. All accidents resulting in a fatality or injury requiring medical treatment will be investigated by one representative of the IBEW and one representative of the Employer. The accident investigation report will be submitted to:
(a) W.C.B.;
(b) Employer; and
(c) IBEW.
Accident Investigation. 1. The Contractor shall cooperate in the conduct of accident investigations which result in recordable injury/illness, property damage, fire, radiation event, and fatality.
2. When a Contractor employee is involved in a serious event or accident, the Contractor shall implement the following actions:
A. Secure the event scene from disturbance and unauthorized entry pending arrival of Battelle Representatives.
B. Keep equipment or articles involved in the event from being operated, moved, or otherwise altered or repaired.

As part of Customer's obligations hereunder AND PURSUANT TO SECTION 3, Customer shall bear the cost of any investigation initiated by Company, Company's insurance carriers or Company's third party adjusters (designated below as Company's "representatives") into any accident of any kind, when such accident occurs during the term of this Agreement, and directly or indirectly involving the Equipment, whether or not such accident involves personal injury or death or damage to the Equipment or other property or any or all of the above. The decision to initiate any such investigation and the scope of any such investigation shall be at the sole discretion of Company or Company's representatives. A Board of Inquiry consisting of the Union Safety and Health Committee, the Union President, a Superintendent and Safety Co-ordinator, shall investigate all accidents involving fatalities and disabling injuries as soon as possible. A copy of the results of such investigation will be furnished to the Union Safety and Health Committee.

The cost of any such investigation that is to be paid by Customer shall include, but not be limited to, attorneys' fees for site inspections, Agreement and document review and interviews with witnesses of any kind, including cost of travel, fees of private investigators for site inspections, obtaining and reproducing agreements and documents and interviews of witnesses, including costs of travel, costs of obtaining and reproducing related agreements and documents, adjusters fees, costs for photography, expert fees including expert forsenic fees, all costs of site inspections and destructive and/or non-destructive testing, as needed, and costs of dismantling, storing and maintaining property, equipment or other items as evidence. These costs shall be invoiced to Customer along with rental fees and other costs incurred under this Agreement and shall be payable according to the terms of this Agreement.

Accident Investigation. Every workplace accident that results in an injury and any near miss that had the potential to cause serious injury will be investigated. The JHSC Union co-chairperson or designee will participate in the investigation.
Accident Investigation. Every injury or near miss which involves or would have involved a worker going to a doctor or hospital must be investigated. The union co-chair or the health and safety rep shall be made available in a timely manner to investigate the accident or incident along with the supervisor or manager of the area.
Accident Investigation. In the event of any accident, loss or casualty event involving any use of the Equipment, whether resulting in property damage or bodily injury or other loss or damage, Customer shall, under Supplier and/or Supplier's insurer's direction, provide, at its sole cost and expense, for the full and complete investigation of any such accident, loss or casualty event. Costs and expenses of any investigation may include, but shall not be limited to, reasonable fees and costs for, lawyers, experts, forensic investigations, adjusters, and inspection, disassembly, transportation and/or storage of the Equipment. The Customer agrees that it will pay all defense costs of investigations conducted by Supplier, or entities working on Supplier’s behalf.
