Investigation Report. The investigative report, findings, response from the employee and any addendum information constitute the completed investigative report and shall become the basis for disciplinary action. The report shall be provided to the employee, or their representative, upon request.
Investigation Report. The Investigator(s) shall prepare the draft and final institutional investigation reports in writing and provide the draft report for comment by respondent in a manner consistent with applicable federal regulations. The final investigation report shall:
(A) describe the nature of the allegations of research misconduct;
(B) describe and document the federal, state or private financial support, including, any grant numbers, grant applications, contracts, and publications listing federal, state or sponsor support;
(C) describe the specific allegations of research misconduct considered in the investigation;
(D) include the institutional policies and procedures under which the investigation was conducted;
(E) identify and summarize the research records and evidence reviewed, and identify any evidence taken into custody, but not reviewed. The report should also describe any relevant records and evidence not taken into custody and explain why.
(F) provide a finding as to whether research misconduct did or did not occur for each separate allegation of research misconduct identified during the investigation, and if misconduct was found,
1. identify it as falsification, fabrication, and/or plagiarism and whether it was intentional, knowing, or in reckless disregard,
2. summarize the facts and the analysis supporting the conclusion and consider the merits of any reasonable explanation by the respondent and any evidence that rebuts the respondent’s explanations,
3. identify the specific federal, state or other grant support for the research;
4. identify any publications that need correction or retraction;
5. identify the person(s) responsible for the misconduct, and
6. list any current support or known applications or proposals for support that the respondent(s) has pending with federal, state or private agencies; and
(G) include and consider any comments made by the respondent and complainant on the draft investigation report. Upon receipt of the report, the VPR shall determine whether the institution accepts the findings in the report. If any finding is not accepted, the finding and the reasons why it is not accepted shall be identified and included in a written report by the VPR. The University shall maintain and provide to ORl upon request all relevant research records and records of the research misconduct proceeding, including results of all interviews and the transcripts or recordings of such interviews.
Investigation Report. Upon its completion, the employer will furnish the employee with a copy of the investigation report on all improper conduct findings. The employer, at the employee’s request, will furnish a copy of any internal investigative report upon its completion.
Investigation Report. The investigator shall prepare a written report to include a statement of the findings of the investigator as to whether the allegations have been substantiated, and as to whether the alleged conduct constitutes hazing, harassment and/or bullying. The report, when referencing student conduct, is a student record and therefore confidential. It will be made available to investigators in the context of a review conducted by the Xxxxxx of the Roman Catholic Diocese of Burlington, or his designee, or investigations of harassment conducted by the Vermont Human Rights Commission or U.S. Department of Education Office of Civil Rights.
Investigation Report. Contractor shall immediately investigate such suspected security incident or breach and provide the County a complete report of the investigation within seven (7) working days.
Investigation Report. The report that will be presented to the case conference following an investigation. NHS – National Health Service PCT – NHS Primary Care Trust Placing Authority – The Local Authority or NHS Body that has commissioned a service from a provider (that may be located outside their Authority).
Investigation Report. The preliminary investigation report will be provided within the timelines outlined in the investi- gation ToR, to the complainant, respondent and Human Resources with the following infor- mation:
Investigation Report. All investigations shall be completed within thirty (30) days and if reasonably feasible prior to the time the Youth involved is released from the Facility and shall be documented in a report. The results of the investigation shall be compiled in an investigation packet and maintained by LCJDC.
Investigation Report. 6.6.5.1 Upon receipt of the notice and both forms of proof required by Section 6.6.1 of this Appendix, SBC-13STATE shall make an investigation as shall be required to report the results to the Collocator. Provided that Collocator has furnished all of the information and proof required by Section 6.6.1 on or before the Bill Due Date, SBC-13STATE will report the results of its investigation within sixty (60) calendar days following the Bill Due Date. If the Collocator is not satisfied by the resolution of the billing dispute under this Section
Investigation Report. 8.9.5.1 Upon receipt of the notice and both forms of proof required by Section 8.9.2 of this Appendix, SBC-13STATE shall make an investigation as shall be required to report the results to the Collocator. Provided that Collocator has furnished all of the information and proof required by Section 8.9.2 on or before the Bill Due Date, SBC-13STATE will report the results of its investigation within sixty (60) calendar days following the Bill Due Date. If the Collocator is not satisfied by the resolution of the billing dispute under this Section 8.9.2 of this Appendix, the Collocator must notify SBC-13STATE in writing within thirty (30) days following receipt of the results of SBC-13STATE’s investigation that it wishes to invoke the informal resolution of billing disputes afforded under Section 8.9.6 of this Appendix.
