Annual Department Plan. Each Department Chairperson or Faculty Representative shall complete and submit an Annual Department Plan to the appro- priate administrator. The Annual Department Plan shall be approved by a majority vote of those Full-Time and Adjunct Faculty in attendance. Discipline specific items will be voted only by the Full-Time and Adjunct Faculty of that discipline. The Plan shall be submitted by the last day of the Winter semester. The Plan shall identify department responsibilities, practices, policies, and procedures including: • Department meeting calendar • Course Selection/Rotation and/or Non-Instructional Faculty duty assignments • Textbooks, Technology, and other Instructional Resources for all modalities • Minimum CompetenciesCourse Fees • Awards/Scholarships/Events • Equipment and Space • Curriculum, Course Design, and ReviewFaculty Support and Development • Assessment and Accreditation • Goal SettingAdvisory CommitteesElection procedure of the Department Chairperson and/or Faculty Representative • Annual Department Plan ReviewApplicable Department designees [e.g. Technical Representative] • Other Relevant Department Responsibilities, Practices, Policies, and/or Procedures Such department practices, policies and procedures as determined by the faculty in the Annual Department Plan shall apply to each and every department or discipline member and are enforceable by the designated administrator. In the absence of any intra-disciplinary agreement on any practices, policies and procedures, the designated administrator will decide. The de- cision will not be grievable. The Department Chairperson or Faculty Representative will inform the designated administrator, in writing, when changes are made to the Annual Department Plan. Those changes to the Annual Department Plan shall take effect the following semester, except for the plan items that can not be materially implemented. Part-Time faculty members will be invited to participate in the processes described above.

  • Department of Health and Human Services An employee notified of a positive controlled substance or alcohol test result may request an independent test of their split sample at the employee’s expense. If the test result is negative, the Employer will reimburse the employee for the cost of the split sample test. An employee who has a positive alcohol test and/or a positive controlled substance test may be subject to disciplinary action, up to and including dismissal, based on the incident that prompted the testing, including a violation of the drug and alcohol free work place rules.

  • Health and Safety Plan 5. Xxxxxx shall prepare and submit under separate cover from the Work Plan, a Health and Safety Plan consistent with Occupational Safety and Health Administration regulations. The Health and Safety Plan shall be submitted to the Department in the form of one electronic copy on compact disk (in .pdf format). Xxxxxx agrees that the Health and Safety Plan is submitted to the Department only for informational purposes.   • Department Review If a mutually acceptable solution has not been reached during Step 1, and the employee intends to pursue the grievance formally, the employee shall submit the grievance in writing on the Employee Grievance Resolution Form to the Department Head with a copy to the Labor Relations Division not later than ten (10) working days after the supervisor’s written response. The Department Head shall consider the grievance and render a written decision within ten (10) working days of receipt of the formal grievance. The written decision shall include a clear and concise statement including the reason(s) for the decision. The Department Head may hold a meeting with the employee to achieve any of the following purposes: 1) to identify why the employee feels there is a grievance and facilitate communication and resolution; 2) to clearly identify issues and areas of agreement/disagreement; and 3) to have the parties present whatever available information/ documentation necessary to fully attempt to resolve the grievance. The employee may be accompanied by his/her shop xxxxxxx during the Department Review, provided that the xxxxxxx is in the same department as the employee, and has been identified by the employee on the Employee Grievance Resolution Form. If the department, in consultation with the Labor Relations Division, determines that the grievance is outside of the Department Head’s authority, or the grievance involves employees working in separate departments, then such grievance shall be submitted to Step 3.

  • COUNTY’S QUALITY ASSURANCE PLAN The County or its agent will evaluate the Contractor’s performance under this Contract on not less than an annual basis. Such evaluation will include assessing the Contractor’s compliance with all Contract terms and conditions and performance standards. Contractor deficiencies which the County determines are severe or continuing and that may place performance of the Contract in jeopardy if not corrected will be reported to the Board of Supervisors. The report will include improvement/corrective action measures taken by the County and the Contractor. If improvement does not occur consistent with the corrective action measures, the County may terminate this Contract or impose other penalties as specified in this Contract.

  • REGISTRATION WITH DEPARTMENT OF REVENUE The CONTRACTOR shall complete registration with the Washington State Department of Revenue and be responsible for payment of all taxes due on payments made under this contract.

  • Health & Safety Committee The Employer agrees to establish a Health & Safety Committee for all employees of the company under the auspices of the Occupational Health and Safety Act (OHSA). The Committee shall be made up of an equal number (not less than one each) of non-bargaining unit and bargaining unit persons who have completed their probationary period. The Union has the right to appoint the members representing the bargaining unit. The Committee shall keep minutes of its meetings and post them in the shop. One copy will be forwarded to the Union office.
