Annual Monitoring. The SAA assures that at least annually it will reevaluate whether a participant meets the level of care required under the State Medicaid plan for coverage of nursing facility services. The SAA understands that this determination may be waived if there is no reasonable expectation of improvement or significant change in the participant’s conditions because of the severity of a chronic condition or the degree of impairment of functional capacity. The SAA assures that it will make reviews conducted in accordance with Sections 460.190 and 460.192 available to the public upon request.
Annual Monitoring. The Authorised Representatives and Key Personnel specified in Schedule 3 shall meet to review the Annual Report, prior to its presentation at the OfS Quality Assessment Committee.
Annual Monitoring. The Freeway School agrees to comply with the annual monitoring of contract compliance required by IC 00-00-00-00. The Freeway School shall maintain all books, documents, papers, and other evidence pertaining to its performance under this Contract and shall make such materials available at their respective offices for inspection by the State Board or its authorized designees.

Annual Monitoring. Commencing in 2004, PacifiCorp shall fund monitoring by the USDA-FS in accordance with the terms of the RRMP as provided in Schedule 17.1.
Annual Monitoring. FC Bank shall provide the Worthington Tax Incentive Review Council, organized under ORC §5709.85 (“Worthington TIRC”), any information reasonably required by the Worthington TIRC to evaluate FC Bank’s compliance with this Agreement, including returns filed pursuant to ORC §5711.02 if requested by the Worthington TIRC. Pursuant to ORC §3735.671(D), FC Bank shall remit an annual monitoring and reporting fee payable to the City at such time as the Worthington TIRC requests information under this §4, with the annual fee equal to the greater of five hundred dollars ($500.00) or one percent (1%) of the amount of taxes exempted hereunder up to a maximum two thousand five- hundred dollars ($2,500.00) annually, for each year throughout the term of this Agreement.
Annual Monitoring. Metro Parks shall prepare an annual contract monitoring report to provide to the Parks Board, the Parks committee of the Metro Council and provide to the District Council members from District 17, 18, and 19 by August 1st of each year of the contract. The monitoring report shall provide information on the (1) construction and upgrades to the park;

Owner shall provide the Plain City Tax Incentive Review Council, organized under ORC §5709.85 (“Plain City TIRC”), any information reasonably required by the Plain City TIRC to evaluate Owner’s compliance with this Agreement, including returns filed pursuant to ORC §5711.02 if requested by the Plain City TIRC. Pursuant to ORC §3735.671(D), Owner shall remit an annual monitoring and reporting fee payable to the Village at such time as the Plain City TIRC requests information under this §4, with the annual fee of five hundred dollars ($500.00) annually, for each year throughout the term of this Agreement.
