Biweekly Reports. (1) A list of employees newly assigned into the representation unit. This report is identified as Actions Report.
(2) A list, identified as Fair Share Report-Employees with Dues Deductions, of employees within the Welfare Non-Supervisory Unit who have a UPE payroll deduction.
(3) A list, identified as Terminated Employees, of employees who have left County service.
(4) A list (including employees' addresses) identified as Agency Shop Report-Employees Without Dues Deduction, who do not have payroll deductions from biweekly earnings.
(5) A list identified as UPE Dues by Representation Unit, which specifies the following information:
(a) Name
(b) Social security number and/or personnel number.
(c) Employment status code (d) Index
Biweekly Reports. (1) A list of employees newly assigned into the representation unit, including mailing addresses, shall be provided to the Union on a biweekly basis.
(2) The County shall provide the Union with a dues and fees deduction report each pay period specifying the following information for each employee within the Operations and Maintenance Unit for which such deduction was made through the County payroll system:
(a) Name
(b) Social security number
(c) Budget unit number associated with the employee's position
(d) Classification code
(e) Employment status code
(f) Amount of gross pay earned in the pay period
(g) Year-to-date deduction amount on a calendar year basis
(h) Amount of the deduction This report includes the grand total of the biweekly deduction amounts for all employees.
Biweekly Reports. (1) Employees newly assigned into the Information Technology and System Professionals Unit.
(2) Employees within the Information Technology and System Professionals Unit who have a UPEC payroll deduction specifying the following information:
(a) Name
(b) Social Security number and/or personnel number.
(c) Employment status code (d) Index

Biweekly Reports. Every two (2) weeks after the closure of the pay period, the Hospital will forward to the Association a dues report and limited membership report.

Biweekly Reports. Borrower agrees to provide to DFS by Noon Eastern Time on Monday of every other week (commencing with the second Monday after the Effective Date) a Borrowing Base certificate as of the end of business on the previous Friday and Borrowing Base Certificate.
