Catastrophic Leave. All certificated employees on active duty with the District are eligible to contribute to the Catastrophic Leave Bank. Participation is voluntary, but requires contribution to the Bank. Only contributors will be permitted to withdraw from the Bank. Note: An employee’s job related illness or injury subject to worker’s compensation coverage under any District-supported long- or short-term disability coverage, with the exception of the first time and those returning from leavethirty (30) day waiting period, shall not be required to contribute 1 day of sick leave/personal necessity eligible for this catastrophic leave to the Bankprovision. 4) Days in 10.13.7 An employee is eligible to use the Catastrophic Leave Bank shall accumulate from year to yearwhen the following conditions are met: a. The employee requesting use of the leave is a participant of the bank. 5) Days shall be contributed b. The employee has submitted a written request that includes a physician’s verification. c. The employee will not receive payment for the time absent due to the Bank and illness or injury from either worker’s compensation or short/long-term District-sponsored disability coverage. d. The employee has exhausted all balances in all paid leave categories. e. The employee has not withdrawn without regard to the daily rate of pay of more than 20 days from the Catastrophic Leave Bank participantwithin the previous 12 months or more than 40 days during a lifetime. 6) f. All certificated employees on active duty 1) The definition of Catastrophic Leave for an employee is a documented illness or injury that is expected to incapacitate the employee for an extended period of time. 2) Eligibility: Teamsters Local 1932 members who have permanent status in the San Bernardino County Superintendent of Schools classified Teamsters bargaining unit. 3) Requests for Catastrophic Leave shall be submitted to the Union via the Teamsters Stewards or assigned Teamsters Business Agent. 4) Once approved, Payroll will review for proper eligibility and/or make the appropriate leave adjustments. 5) Participation in the Catastrophic Leave Program is voluntary. Catastrophic Leave. All This agreement is intended to assist an eligible employee1 “employee” in the event the employee suffers a severe, incapacitating illness or injury which is expected to continue for a period of at least five (5) days or more, which prevents the employee from performing his or her duties. Catastrophic Leave. All certificated employees on active duty When an eligible employee suffers a catastrophic illness or injury, and the eligible employee has exhausted all accrued leaves as a result of the illness/injury, then the eligible employee may file a request for donations of leave with the District are Association.
