CONTRACTORS AND/OR PROVIDERS RESPONSIBILITIES. A. Prior to and during the provision of any OMFS service pursuant to this Agreement, Contractor shall ensure that each Provider has the qualifications noted in Exhibit A-1, Service Specifications. Each Provider shall possess and maintain in good standing all licenses, permits, and training certification requirements that are specified in that exhibit or elsewhere in this Agreement. Contractor and its Providers are required to provide CDCR/CCHCS with true and accurate copies of any required training, license(s), permit(s), certification(s) and any other required documents as stated above and/or as required by law, prior to providing OMFS services under this Agreement. Only Provider(s) who meet these requirements are authorized to provide OMFS services. Any service by a Provider not meeting these requirements, when the service was provided, will be deemed unauthorized and not entitled to compensation and/or reimbursement.

CONTRACTORS AND/OR PROVIDERS RESPONSIBILITIES. Under the direction of the institution CEO/CME or designee, or facility CMO or designee, Contractors and/or Providers shall provide services as permitted within the scope of practice for Prosthetic and Orthotic services in accordance with institution/facility policies and procedures.
CONTRACTORS AND/OR PROVIDERS RESPONSIBILITIES. Services and all requirements shall be provided by the Contractor and/or Provider for the contracted services, at the request of the institution CEO/CME or designee, or facility CMO or designee. The services shall include, but are not limited to the following: Contractor and/or Provider must be able to adequately provide services to all institutions/facilities awarded on Exhibit B-1, Bid Rate Sheet, and meet the requirements specified in Exhibit A–1, Service Specifications. Contractor and/or Provider must provide the State with proof of any required training, licenses, permits, certifications and any other required documents as stated in Exhibit A–1, Service Specifications and as required by law, when providing services under this Agreement. Services will be performed only by Contractors and/or Providers who meet the requirements outlined in this Agreement. Refer to Exhibit A-1, Service Specifications for Contractors and/or Providers Responsibilities.

  • PROVIDER’S RESPONSIBILITIES A Provider (HUB or non-HUB) must perform a minimum of 30% of the contract with its employees (as defined by the Internal Revenue Service). The contract is subject to the HSP Good Faith Effort Requirements.

  • Contractor's responsibility for subcontractors The Contractor shall be as fully responsible to the City for the acts and omissions of its subcontractors, and of persons either directly or indirectly employed by them, as Contractor is for the acts and omissions of persons it directly employs. Contractor shall cause appropriate provisions to be inserted in all subcontracts relating to this work, to bind all subcontractors to Contractor by all the terms herein set forth, and insofar as applicable to the work of subcontractors and to give Contractor the same power regarding termination of any subcontract as the City may exercise over Contractor under any provisions of this contract. Nothing contained in this contract shall create any contractual relation between the subcontractor and the City or between any subcontractors.

  • CONTRACTOR'S RESPONSIBILITY Nothing herein shall be construed as limiting in any way the extent to which contractor/renter may be held responsible for damages resulting from contractor/renter’s operations, acts, omissions or negligence. Insurance coverage obtained in the minimum amounts specified above shall not relieve contractor/renter of liability in excess of such minimum coverage, nor shall it preclude the fair from taking other actions available to it under contract documents or by law, including, but not limited to, actions pursuant to contractor/renter’s indemnity obligations. The contractor/renter indemnity obligations shall survive the expiration, termination or assignment of this contract.

  • Customer’s Responsibilities 9.1 If and to the extent applicable or under the control of the Customer, Customer shall provide complete and accurate information regarding requirements for the Project and the Site(s), including, without limitation, constraints, space requirements, underground or hidden facilities and structures, and all applicable drawings and specifications.

  • THE OWNER'S RESPONSIBILITIES 5.1 The Owner shall provide general requirements and description for the Project (budget limitations and required delivery schedules, etc.).

  • EMPLOYEES, SUBCONTRACTORS AND AGENTS All employees, Subcontractors, or agents of the Contractor performing work under the Contract must be trained staff or technicians who meet or exceed the professional, technical, and training qualifications set forth in the Contract or the Purchase Order, and must comply with all security and administrative requirements of the Authorized User that are communicated to the Contractor. The Commissioner and the Authorized User reserve the right to conduct a security background check or otherwise approve any employee, Subcontractor, or agent furnished by Contractor and to refuse access to or require replacement of any personnel for cause based on professional, technical or training qualifications, quality of work or change in security status or non-compliance with Authorized User’s security or other requirements. Such approval shall not relieve the Contractor of the obligation to perform all work in compliance with the Contract or the Purchase Order. The Commissioner and the Authorized User reserve the right to reject and/or bar from any facility for cause any employee, Subcontractor, or agent of the Contractor.
