Description of Data. Station will track the number of children demonstrating gains in school readiness including but not limited to:
Description of Data. The types of data to which this agreement applies are Hospital, Rehabilitation facility and Community Partners such as Skilled Nursing Facility (SNF), Providers, Physicians, Home Health Agency (HHA), Palliative Care, Hospice, Health Clinics, Health Centers, Pharmacies/Pharmacists, Long Term Acute Care (LTAC), and IRF utilization data emphasizing readmission/admission rates.

Station will track the number of children demonstrating gains in school readiness including but not limited to: Social Relationships Self-Concept and Self-Efficacy Emotional and Behavioral Health Literacy Skills Alphabet Knowledge Early Writing Numeracy Skills Numbers and Operations Station will utilize ________________ (“instrument”) found at ________________ to collect this information. Sponsor will use this information to fulfill its contractual responsibilities to report on performance to the Corporation for National and Community Service (CNCS). Description of Data i. Contractors collect various data elements associated with prevention programming and service delivery. The Data will be provided by contractors on a monthly and a quarterly basis and entered into Xxxxxxx.
ii. Data Use Purpose. (if more detail from purpose of overarching agreement is needed (i.e., more restrictions, etc.) The data is used by state and local providers for contract management, program monitoring and to evaluate outcomes.

2.1.1. Please note: Schedule C and D are not attached hereto but are incorporated by reference and are available upon request from the HCA Contract Manager listed on the Cover Page of this Contract.
Description of Data. The data elements to be exchanged under this Agreement (the “Data”) include:
1. the number of students enrolled exclusively in the institution by means of distance education, disaggregated by state, territory, or district in which students are located, including the Home State; and
2. the numbers of students engaged in OOSLP (e.g., experiential learning placements, rotations, clinicals, internships, student teaching, etc.), disaggregated by state/territory and two-digit Classification of Instructional Programs (CIP) codes as assigned by the U.S. Department of Education. The data do not constitute protected individual-level data under FERPA and no personally identifiable information is shared. Data are shared only in the manner reported by the institution, with no further disaggregation. Each institution participating in XXXX shall annually report this data to NC-XXXX. The data shall be reported in the spring following the due date for institutions to make their fall distance education enrollment reports to the federal government’s Integrated Postsecondary Education Data System (IPEDS). The current IPEDS Data Collection Schedule is available on this IPEDS website: xxxxx://xxxxxxx.xxxx.xx.xxx/IPEDS/ Institutions not participating in federal Title IV student assistance programs, and, therefore not reporting to IPEDS, shall nevertheless report this data to NC- XXXX using the same schedule. The Data Reporting Handbook for Institutions provides detailed information about EDEE and OOSLP data reporting.
