DESIGNATED LEAD AGENCY. Contractor Partner Agencies agrees that Boys and Girls Clubs of Tustin shall serve as the designated lead agent on behalf of the CONTRACTOR, with authority to present claims to COUNTY on behalf of each of the Contractor Partner Agencies for services delivered by each of them pursuant to this Agreement. As designated lead agent, Boys and Girls Clubs of Tustin shall receive the claims from each of the other Contractor Partner Agencies on a monthly basis and shall submit these claims, along with its own monthly claim, pursuant to Paragraph 21 herein. Claims submitted to COUNTY by the designated lead agent shall clearly identify the services that were performed by Contractor Partner Agencies. Each of the Contractor Partner Agencies agrees that COUNTY’s disbursement of payment to the designated lead agent shall satisfy COUNTY’s payment obligation under this Agreement. DESIGNATED LEAD AGENCY. 8 Each of the Contractor Partner Agencies agrees that the City of Xxxxxxx 9 (STNCity) shall serve as the designated lead agent on behalf of the CONTRACTOR, with 10 authority to present claims to COUNTY on behalf of each of the Contractor Partner Agencies for 11 services delivered by each of them pursuant to this Agreement. As designated lead agent, STN,the 12 City shall receive the claims from each of the other Contractor Partner Agencies on a monthly 13 basis and shall submit these claims, along with its own monthly claim, pursuant to Paragraph 2021 14 herein. Claims submitted to COUNTY by the designated lead agent shall clearly identify the 15 services that were performed by Contractor Partner Agencies. Any and all payments to be made 16 by COUNTY pursuant to this Agreement shall be made payable to the designated lead agent. As designated lead agent, 5 Mission Hospital, shall submit claims on a monthly basis pursuant to Paragraph 21 herein. Claims 6 submitted to COUNTY by the designated lead agent shall clearly identify the services that were 7 performed. Any and all payments to be made by COUNTY pursuant to this Agreement shall be 8 made payable to the designated lead agent. 9 As the designated lead agent, Mission Hospital shall also be responsible for 10 activities that include but are not limited to the following:

As designated lead agent, The Raise Foundation shall 8 receive the claims from each of the other Contractor Partner Agencies on a monthly basis and shall 9 submit these claims, along with its own monthly claim, pursuant to Paragraph 21 herein. Claims 10 submitted to COUNTY by the designated lead agent shall clearly identify the services that were 11 performed by Contractor Partner Agencies. Any and all payments to be made by COUNTY 12 pursuant to this Agreement shall be made payable to the designated lead agent. The designated 13 lead agent shall thereafter disburse payment as appropriate to the Contractor Partner Agencies.

Claims submitted to COUNTY by the designated lead agent shall clearly identify the services that were performed. Any and all payments to be made by COUNTY pursuant to this Agreement shall be made payable to the designated lead agent. 20.2.1 Oversight of FRC services;
20.2.2 Employment and supervision of the FRC Coordinator;
20.2.3 Employment and/or oversight of the Information and Referral Specialist;
20.2.4 Employment and/or oversight of the Community Engagement Coordinator;
20.2.5 Establishing and facilitating a monthly FRC meeting with Contractor Partner Agencies and ensuring meetings minutes are documented;
20.2.6 Coordinating weekly Case Management Team (CMT) meetings; 20.2.7
DESIGNATED LEAD AGENCY. Both OneOC and CHIOC agree that OneOC shall serve as the designated lead agent on behalf of the CONTRACTOR, with authority to present claims to COUNTY on behalf of the Contractor Partner Agency for services delivered by each of them pursuant to this Agreement. As designated lead agent, OneOC, shall receive the claims from CHIOC on a monthly basis and shall submit these claims, along with its own monthly claim, pursuant to Subparagraph 21.3.1 herein. Claims submitted to COUNTY by the designated lead agent shall clearly identify the services that were performed by CHIOC. Any and all payments to be made by COUNTY pursuant to this Agreement shall be made payable to the designated lead agent. The designated lead agent shall thereafter disburse payment as appropriate to CHIOC. CHIOC agrees that COUNTY’s disbursement of payment to the designated lead agent shall satisfy COUNTY’s payment obligation under this Agreement. As the designated lead agent, OneOC shall also be responsible for, at a minimum, facilitating CONTRACTOR meetings, collecting documentation for invoices and outcome measurements from CHIOC, that include but are not limited to the following:
20.2.1 Collecting and maintaining complete documentation for invoices.
20.2.2 Maintaining complete and accurate records of all financial and outcome measurement data.
20.2.3 Overseeing the collection, maintenance, and management of required data including outcome measurements.
20.2.4 Facilitating collaborative activities, services, and programs to ensure effective service delivery.
20.2.5 Ensuring required documentation (e.g., insurance certificates, copies of resumes/applications, independent audits) is current.
20.2.6 Collecting information and generating a monthly outreach activity calendar.
DESIGNATED LEAD AGENCY. 5 EachCommunity Action Partnership of the Contractor Partner Agencies 6 agrees that the City of Westminster (CityOrange County (Community Action Partnership) shall 7 serve as the designated lead agent on behalf of the CONTRACTOR, with authority to present 8 claims to COUNTY on behalf of each of the Contractor Partner Agencies for services delivered 9 by each of them pursuant to this Agreement. As designated lead agent, City,Community Action 10 Partnership shall receive thesubmit claims from each of the other Contractor Partner Agencies on 11 a monthly basis and shall submit these claims, along with its own monthly claim, pursuant to 12 Paragraph 2022 herein. Claims submitted to COUNTY by the designated lead agent shall clearly 13 identify the services that were performed by Contractor Partner Agencies.. Any and all payments 14 to be made by COUNTY pursuant to this Agreement shall be made payable to the designated lead
DESIGNATED LEAD AGENCY. 8 Each of the Contractor Partner Agencies agrees that the City of Westminster 9 (CityLatino Center for Prevention and Action in Health and Welfare DBA Latino Health Access 10 (LHA) shall serve as the designated lead agent on behalf of the CONTRACTOR, with authority 11 to present claims to COUNTY on behalf of each of the Contractor Partner Agencies for services 12 delivered by each of them pursuant to this Agreement. As designated lead agent, City,LHA shall 13 receive the claims from each of the other Contractor Partner Agencies on a monthly basis and shall 14 submit these claims, along with its own monthly claim, pursuant to Paragraph 2021 herein. Claims 15 submitted to COUNTY by the designated lead agent shall clearly identify the services that were 16 performed by Contractor Partner Agencies. Any and all payments to be made by COUNTY 17 pursuant to this Agreement shall be made payable to the designated lead agent.
