DESIGNATED LEAD AGENCY. 26 Each of the Contractor Partner Agencies agrees that Human Options, Inc. 27 (Human Options) shall serve as the designated lead agent on behalf of the CONTRACTOR, with 28 authority to present claims to COUNTY on behalf of each of the Contractor Partner Agencies for 1 services delivered by each of them pursuant to this Agreement. As designated lead agent, 2 HO,Human Options shall receive thesubmit claims from each of the other Contractor Partner 3 Agencies on a monthly basis and shall submit these claims, along with its own monthly claim, 4 pursuant to Paragraph 2021 herein. Claims submitted to COUNTY by the designated lead agent 5 shall clearly identify the services that were performed by Contractor Partner Agencies.. Any and 6 all payments to be made by COUNTY pursuant to this Agreement shall be made payable to the 7 designated lead agent. The designated lead agent shall thereafter disburse payment as appropriate 8 to the Contractor Partner Agencies. Each of the Contractor Partner Agencies agrees that 9 COUNTY’s disbursement of payment to the designated lead agent shall satisfy COUNTY’s 10 payment obligation under this Agreement. 11 As the designated lead agent, HOHuman Options shall also be responsible 12 for activities that include, but are not limited to, the following:
13 19.2. 120.2.1 Oversight of FRC services;
14 19.2. 220.2.2 Employment and supervision of the FRC Coordinator;
DESIGNATED LEAD AGENCY. 17 Each of the Contractor Partner Agencies agrees that The Raise Foundation 18 (RF)Mission Hospital shall serve as the designated lead agent on behalf of the CONTRACTOR, 19 with authority to present claims to COUNTY on behalf of each of the Contractor Partner Agencies 20 for services delivered by each of them pursuant to this Agreement. As designated lead agent, 21 RF,Mission Hospital shall receive thesubmit claims from each of the other Contractor Partner 22 Agencies on a monthly basis and shall submit these claims, along with its own monthly claim, 23 pursuant to Paragraph 2022 herein. Claims submitted to COUNTY by the designated lead agent 24 shall clearly identify the services that were performed by Contractor Partner Agencies.. Any and 25 all payments to be made by COUNTY pursuant to this Agreement shall be made payable to the 26 designated lead agent. The designated lead agent shall thereafter disburse payment as appropriate 27 to the Contractor Partner Agencies. Each of the Contractor Partner Agencies agrees that 28 COUNTY’s disbursement of payment to the designated lead agent shall satisfy COUNTY’s 1 payment obligation under this Agreement.
DESIGNATED LEAD AGENCY. 17 19.1 Each of the Contractor Partner Agencies agrees that the City of 18 Xxxxxxx (STN) shall serve as the designated lead agent on behalf of the 19 CONTRACTOR, with authority to present claims to COUNTY on behalf of each of 20 the Contractor Partner Agencies for services delivered by each of them 21 pursuant to this Agreement. As designated lead agent, STN, shall receive the 22 claims from each of the other Contractor Partner Agencies on a monthly basis 23 and shall submit these claims, along with its own monthly claim, pursuant to 24 Paragraph 20 herein. Claims submitted to COUNTY by the designated lead agent 25 shall clearly identify the services that were performed by Contractor Partner 26 Agencies. Any and all payments to be made by COUNTY pursuant to this 27 Agreement shall be made payable to the designated lead agent. The designated 28 lead agent shall thereafter disburse payment as appropriate to the Contractor 1 Partner Agencies. Each of the Contractor Partner Agencies agrees that 2 COUNTY’s disbursement of payment to the designated lead agent shall satisfy 3 COUNTY’s payment obligation under this Agreement.
4 19.2 As the designated lead agent, STN shall also be responsible for at 5 a minimum facilitating CONTRACTOR meetings, collecting documentation for 6 invoices, and outcome measurements from each CONTRACTOR Partner Agency, and 7 maintaining complete and accurate records of all financial and outcome 8 measurement data on behalf of CONTRACTOR activities that include but are not 9 limited to the following:
10 19.2.1 Oversight of FRC services;
11 19.2.2 Employment and supervision of the FRC Coordinator;
12 19.2.3 Facilitating established meetings for Contractor Partner 13 Agencies and generating meeting minutes;
14 19.2.4 Coordinating a minimum of weekly case management 15 meetings;
16 19.2.5 Collecting and maintaining complete documentation for 17 invoices from Contractor Partner Agencies;
18 19.2.6 Overseeing the collection, maintenance, and management of 19 FRC data including outcome measurements from Contractor Partner Agencies;
20 19.2.7 Generating monthly reports (i.e. Service Grids) in 21 accordance with Paragraph 38 of this Agreement and Exhibit Paragraph 9 for 22 submission to COUNTY;
23 19.2.8 Reimbursing FaCT-funded Contractor Partner Agencies for 24 FaCT-funded services rendered prior to invoicing COUNTY;
25 19.2.9 Generating modification requests on the FRC's behalf for 26 submission to COUNTY;

Children’s Bureau of Southern California (Children’s Bureau) shall serve as the designated lead agent with authority to present claims to COUNTY for services delivered pursuant to this Agreement. As designated lead agent, Children’s Bureau, shall submit claims on a monthly basis pursuant to Paragraph 21 herein. Claims submitted to COUNTY by the designated lead agent shall clearly identify the services that were performed. Any and all payments to be made by COUNTY pursuant to this Agreement shall be made payable to the designated lead agent. DESIGNATED LEAD AGENCY. 8 Each of the Contractor Partner Agencies agrees that the City of Westminster 9 (CityLatino Center for Prevention and Action in Health and Welfare DBA Latino Health Access 10 (LHA) shall serve as the designated lead agent on behalf of the CONTRACTOR, with authority 11 to present claims to COUNTY on behalf of each of the Contractor Partner Agencies for services 12 delivered by each of them pursuant to this Agreement. As designated lead agent, City,LHA shall 13 receive the claims from each of the other Contractor Partner Agencies on a monthly basis and shall 14 submit these claims, along with its own monthly claim, pursuant to Paragraph 2021 herein. Claims 15 submitted to COUNTY by the designated lead agent shall clearly identify the services that were 16 performed by Contractor Partner Agencies. Any and all payments to be made by COUNTY 17 pursuant to this Agreement shall be made payable to the designated lead agent. As designated lead agent, CAPOC,Community Action 27 Partnership shall receive thesubmit claims from each of the other Contractor Partner Agencies on 28 a monthly basis and shall submit these claims, along with its own monthly claim, pursuant to 1 Paragraph 21 herein. DESIGNATED LEAD AGENCY. 4 6 Contractor Partner Agency for services delivered by each of them pursuant to this Agreement. As 7 designated lead agent, OneOC, shall receive the claims from CHIOC on a monthly basis and shall 8 submit these claims, along with its own monthly claim, pursuant to Subparagraph 1.1.1.1 herein. 9 Claims submitted to COUNTY by the designated lead agent shall clearly identify the services that 10 were performed by CHIOC. Any and all payments to be made by COUNTY pursuant to this 11 Agreement shall be made payable to the designated lead agent. The designated lead agent shall 12 thereafter disburse payment as appropriate to the CHIOC.
