Disputing an Evaluation. Should a House Officer wish to dispute a rotation evaluation or an overall performance evaluation, he/she shall be given the opportunity, upon request, to meet with the Program Director. Such request must be submitted in writing to the Program Director within thirty (30) days of receipt by/delivery to by the House Officer of the evaluation. If, after fourteen (14) days of the filing of the written request the House Officer is not satisfied with the action taken on the matter, he/she shall have the opportunity to meet with the Chief Medical Officer. The House Officer shall have the right to bring to these meetings a representative of the Union, or at his/her discretion, an attending chosen by the House Officer.

Disputing an Evaluation. A HSO may dispute an evaluation by sending a letter to the program director within 25 business days of the resident receiving the evaluation. The letter shall state what aspects of the evaluation are being disputed and identify the remedy being sought by the HSO. If after 10 business days the resident is not satisfied with the action taken, the resident can request a meeting with the Director of GME. The Director of GME shall meet with the resident within 10 business days of receiving this request as a final step in the dispute.

  • Final Evaluation IC must submit a final report and a project evaluation to the Arts Commission within thirty (30) days after the completion of the Services. Any and all unexpended funds from IC must be returned to City no later than sixty (60) days after the completion of the Services.

  • Annual Evaluation The Partnership will be evaluated on an annual basis through the use of the Strategic Partnership Annual Evaluation Format as specified in Appendix C of OSHA Instruction CSP 00-00-000, OSHA Strategic Partnership Program for Worker Safety and Health. Xxxxxxxxx & Xxxxxx will be responsible for gathering required participant data to evaluate and track the overall results and success of the Partnership. This data will be shared with OSHA. OSHA will be responsible for writing and submitting the annual evaluation.

  • BID EVALUATION The Commissioner reserves the right to accept or reject any and all Bids, or separable portions of Bids, and waive technicalities, irregularities, and omissions if the Commissioner determines the best interests of the State will be served. The Commissioner, in his/her sole discretion, may accept or reject illegible, incomplete or vague Bids and his/her decision shall be final. A conditional or revocable Bid which clearly communicates the terms or limitations of acceptance may be considered, and Contract award may be made in compliance with the Bidder’s conditional or revocable terms in the Bid.
