District ProgramU. The District shall provide life, hospital, medical, dental, and vision insurance selected from the following carriers: CALPERS MAJOR MEDICAL HEALTH PLANS: Currently including ANTHEM HMO SELECT, ANTHEM TRADITIONAL, BLUE SHIELD A+, HEALTH NET SALUD Y MAS, HEALTH NET SMARTCARE, XXXXXX PERMANENTE, PERS CARE, PERS CHOICE, PERS SELECT, PORAC, SHARP, UNITED HEALTHCARE VALUE, VISION SERVICE PLAN DELTACARE and DELTA DENTAL PPO PLAN LIFE INSURANCE - $75,000 (MetLife) 8.B. D U istrict Contribution:U The annual District contribution toward the premiums (Section 8.A) for each eligible full-time unit member shall be as follows: First contract year (2019-20): Single Party Tier PERS Major Medical Health Plans, Dental, Vision, and Life $12,159* Two Party Tier PERS Major Medical Health Plans, Dental, Vision, and Life $14,593* Family Party Tier PERS Major Medical Health Plans, Dental, Vision, and Life $16,119* *If a unit member's total premiums cost less than $12,159, they will receive the difference between the cost of their premiums and $12,159 as cash back. If a unit member's total premiums cost more than $12,159, they will not receive cash back.
