Extended Health Benefit Plan. The Employer shall contribute 100% of the premium of this plan. (i) Total lifetime coverage will be unlimited. (ii) Hearing aid benefit claims will be to a maximum of $600 every five years. (iii) The Employer will provide an eyewear program providing $250 per family member every two (2) years.
