Family Premium Clause Samples

The Family Premium clause defines the additional insurance premium required to extend coverage to an insured's family members under a policy. Typically, this clause specifies which family members are eligible—such as spouses and dependent children—and outlines the calculation or flat fee for including them. Its core function is to provide a clear mechanism for policyholders to secure broader protection for their families, ensuring that all covered individuals are properly accounted for and that the insurer receives appropriate compensation for the increased risk.
Family Premium. Employees electing to obtain dependent coverage under a Family coverage election under the medical plan(s) will pay 25% of the premium cost of the dependent portion of Family coverage in addition to any applicable employee premium cost discussed in subsection A above, and the Unified Government shall pay the remaining contribution cost for the least expensive medical plan provided or administered by a major reputable carrier recommended by the Joint Committee and approved by the Unified Government Administrator.
Family Premium. Eighty percent (80%) of the actual monthly premium for all bargaining unit members hired prior to July 1, 2005 who are regularly scheduled to work at least fifteen (15) hours per work; eighty percent (80%) of the actual monthly premium for all bargaining unit members hired on or after July 1, 2005 who are regularly scheduled to work at least thirty-two (32) hours per week; and fifty percent (50%) of the actual monthly premium for all bargaining unit members hired on or after July 1, 2005 who are regularly scheduled to work between 20 and 31.99 hours per week. In the event of an increase in the premium, the Board will pay the same percent, as listed above, of such increase.
