To meet the GPRA requirements, SAMHSA must collect performance data (i.e., “GPRA data”) from grantees. The performance requirements for XXXXXX’s CMHI grant program are described in Section I-2.6 of this document under “Data Collection and Performance Measurement.”

Government Performance and Results Act. (GPRA) Under the Government Performance and Results Act of 1993 (5 U.S.C. 306, 31 U.S.C. 1114- 1119, “GPRA”) the NSF is required to report on the Federal Performance Goals for Facilities. This may include the collection and submission of specific data related to the NSF GPRA requirements. The awardee will be required to submit estimates of the relevant performance metrics early in the fiscal year (by December 31) and actuals shortly after the fiscal year end (by October 31). Any and all facilities with an annual budget exceeding $1,000,000 must report on their operations activities; and any and all construction/upgrade projects that exceed a total project cost of a specified threshold must report on their construction/upgrade activities. Therefore, the awardee will be required to submit reports related to the GPRA performance goals to the cognizant NSF Program Officer. Government Performance and Results Act. (GPRA): Xxxxxx Health System will perform and submit assessments for all participants who are considered engaged in treatment and recovery support services. Personnel Xxxxxxxx Xxxx, MD, will devote 20% annual effort to the project. Xx. Xxxx is Associate Professor, Family and Community Medicine and Chief of Staff, Ambulatory Care Services at Xxxxxx Health System. Xx. Xxxx’x effort is included in the budget as a subcontract to the University of Texas Health Science Center at Houston. Xx. Xxxx will treat patients and supervise project personal at Xxxxxx Health System’s Baytown Health Center. Xxxxxxxx XxXxx, MBA, RN, will devote 25% annual effort to the project. Xx. XxXxx is Director, Nursing Informatics and Strategic Initiatives in Ambulatory Care Services at Xxxxxx Health System. Xx. XxXxx will supervise the project’s nurse care managers and will oversee implementation of the screening tool in EPIC. To Be Named, RN, will commit 100% annual effort to the project and serve as Nurse Care Manager. An additional Nurse Care Manager will be hired six months into the project to accommodate full patient enrollment. This role is included in the budget as 0.5 FTE. Xxxx Xxxx, Lead EPIC Analytics Developer, will commit 10% annual effort to the project. Xx. Xxxx will complete all project-related data collection, analysis, and reporting.

Related to Government Performance and Results Act

  • Quarterly Contractor Performance Reporting Customers shall complete a Contractor Performance Survey (Exhibit I) for each Contractor on a Quarterly basis. Customers will electronically submit the completed Contractor Performance Survey(s) to the Department Contract Manager no later than the due date indicated in Contract Exhibit D, Section 17, Additional Special Contract Conditions. The completed Contractor Performance Survey(s) will be used by the Department as a performance-reporting tool to measure the performance of Contractors. The Department reserves the right to modify the Contractor Performance Survey document and introduce additional performance-reporting tools as they are developed, including online tools (e.g. tools within MyFloridaMarketPlace or on the Department's website).

  • Ongoing Performance Measures The Department intends to use performance-reporting tools in order to measure the performance of Contractor(s). These tools will include the Contractor Performance Survey (Exhibit H), to be completed by Customers on a quarterly basis. Such measures will allow the Department to better track Vendor performance through the term of the Contract(s) and ensure that Contractor(s) consistently provide quality services to the State and its Customers. The Department reserves the right to modify the Contractor Performance Survey document and introduce additional performance-reporting tools as they are developed, including online tools (e.g. tools within MFMP or on the Department's website).

  • COMPLIANCE AND PERFORMANCE EVALUATION Two (2) Mandatory Compliance and Performance Evaluation Meetings shall be conducted during each Term of this Agreement. Additional meetings may be scheduled at the discretion of Department to ensure Concessionaire’s effectiveness and compliance. The meetings shall review all aspects of the Concession Operation, ensuring that quality public services are being provided on a continuing basis in accordance with the Bid Specifications and this Agreement, that operational problems/concerns are addressed on a timely basis, and that all terms and conditions are clearly understood. The meetings shall be held on site with Department-designated State Park Service staff representative(s), the on-site concession manager, and a management/supervisory representative of Concessionaire’s firm. A report form shall be utilized to document the meeting, and to identify any deficiencies and the corrective action required. A copy of the completed report form shall be provided to the on- site concession manager or the management/supervisory representative of Concessionaire’s firm and shall be attached to and made a part of this Agreement. The Mandatory Compliance and Performance Evaluation Meetings shall be held as follows: • Meeting #1 - Prior to commencement of the Period of Operation or Memorial Day, whichever comes first. • Meeting #2 - Within ten (10) calendar days after the last approved day of the Period of Operation.

  • Employee Performance Review When a formal review of an employee’s performance is made, the employee concerned shall be given an opportunity to discuss, sign and make written comments on the review form in question and the employee is to receive a signed copy to indicate that its contents have been read. An employee shall be entitled to a minimum of two (2) work days to review the performance review prior to providing any response to the Employer, verbally or in writing, with respect to the evaluation.
