PERFORMING AGENCY RESPONSIBILITIES. Specific deliverables for the Mental Health Program for Veterans (MHPV) are implemented through this Statement of Work (SOW) which enhances access to, awareness of, and connection to services and resources for trauma-affected military service members, veterans, and their families (SMVF). Texas Health and Human Services Commission (HHSC) administers this SOW in coordination with the Texas Veterans Commission (TVC). Through this SOW, local mental health authorities/local behavioral health authorities (LMHAs/LBHAs) employ or contract for a Peer Service Coordinator (PSC) to implement local networks of the Military Veteran Peer Network (MVPN), which: 1. Provide direct peer-to-peer services to trauma-affected military SMVF; 2. Train MVPN peer volunteers, the SMVF community, and communities at-large on the issues and impacts of military-related traumas, using TVC-developed curriculum approved by HHSC; 3. Assist SMVF to identify and access appropriate, culturally competent mental health services, including access to licensed mental health professionals (LMHPs) and supportive services within their service coverage area; 4. Make referrals to community-based resources, programs, and activities, which provide health-focused supportive services; and 5. Provide peer-to-peer services to justice-involved veterans (JIV) who may be impacted by military-related traumas. Contractor shall achieve the objectives of the MHPV as outlined in Section I.A.1.-6. (below) by hiring or contracting for services from at least one MVPN PSC. LMHA/LBHA-employed or - contracted PSCs are trained and certified by TVC’s Veteran Mental Health Department (VMHD) using curriculum approved by HHSC to achieve the deliverables of this SOW. The PSC shall serve 40 hours per week, exclusively to achieve the objectives of the program as delineated in Section I.A.1.-6. (below). The PSC’s base of operations should be a site selected by agreement between Contractor and the PSC. As a best practice, the site should: 1. Be a Peer Networking Center, which is a facility that the PSC determines SMVF would likely be comfortable visiting; and 2. Be near or co-located with one or more of the following: County Veteran Service Officers (CVSOs), TVC representatives, or Texas Workforce Commission (TWC) Veterans Resource & Referral Specialists.

PERFORMING AGENCY RESPONSIBILITIES. 1. PROGRAM IMPLEMENTATION A. Performing Agency will provide integrated opioid response prevention, treatment, and recovery services and activities: a. to the identified primary population where the target population is located as approved by HHSC. B. The IFPOR provider site must be actively serving persons within the target population no later than three months following the date of the interagency contract agreement execution.

Assess previous SUDP distribution efforts to inform future SUDP dissemination and evaluation strategies. a. Previous data will be provided by System Agency. b. In the first 60 days post-contract start date or September 29, 2019, whichever is sooner, compile previous data, create, and submit a Baseline Report for System Agency review. The report will include previous methods for SUDP distribution, end- user groups who received SUDPs, SAMHSA Center for Substance Abuse Prevention (CSAP) strategies implemented along with SUDP distribution, and categorized Prevention Programs based on their served target population. This information will also help the Performing Agency to estimate the need for SUDPs (quantity and size [small, medium, or large]) for each distributor. i. The CSAP strategies are Information Dissemination, Prevention Education, Positive Alternatives, Environmental Strategies, Community-Based Process, and Identification of Problems and Referral to Services. ii. Additional information about the prevention and CSAP strategies can be found in the Substance Abuse and Mental Health Services Administration, Focus on Prevention, HHS Publication No. (SMA10–4120). Collaborate with a manufacturer of SUDPs to discuss the specific needs for the project as they relate to distribution and data collection. 4. Coordinate the provision of SUDPs to distributors (e.g., Prevention Programs, physician offices, hospitals) with data collection questionnaires attached to each unit. 5. Cover all costs associated with SUDP delivery to distributors. 6. After estimating need and target population, contact each distributor, by any means necessary to discuss distribution method, prevention strategies, and physical mailing address for the SUDP order. PERFORMING AGENCY RESPONSIBILITIES. A. Pilot program enrollment is limited to one year and may only be extended beyond one year as recommended by the Intensive Transition Team (ITT) and approved by HHSC. B. Pilot programs shall accept referrals for individuals whose designated county of residence is outside of Contractor’s local service area C. Pilot program participants shall: 1. Be age 18 years of age or older; 2. Have a history of serious mental illness; 3. Have a history of multiple hospitalizations or long-term care; 4. Not be a danger to himself/herself, others, or property; 5. Voluntarily consent to the housing option. If the individual has a guardian, then guardian must consent to the housing option. 6. Voluntarily consent to engage in intensive services with the LMHA/LBHA; and 7. Have ITT and HHSC approval prior to the individual’s placement D. Prior to enrollment in the Pilot program, participants must receive a comprehensive assessment that includes, but is not limited to: 1. Financial and benefit eligibility assessment; 2. Uniform Assessment, including the Adult Needs and Strengths Assessment (ANSA) and Community Data, and, at minimum, every 180-days thereafter; 3. The Quality of Life Scale (QOLS), and, at minimum, every 180-days thereafter; 4. The Social and Occupational Functioning Scale (SOFS), and, at minimum, every 180- days thereafter; and 5. Diagnostic profile that includes all applicable diagnoses (e.g., psychiatric, substance use, physical conditions, and intellectual or developmental disabilities). E. Pilot programs shall use an ITT to plan for and complete the step-down/transition process. The ITT shall include, as applicable or determined necessary based on clinical presentation and client preference, state hospital staff, LMHA/LBHA continuity of care staff, Medicaid Managed Care Organization staff, a peer support specialist, a substance use counselor, the participant, and the participant’s family or legally authorized representative. The ITT shall: 1. Provide pre-transition services while the individual is still in the state hospital for up to six months; 2. Develop and facilitate implementation of the Individual Transition Plan (ITP), which must address clinical and housing needs; and 3. Participate in the individual’s care throughout enrollment in the Pilot program; F. Pilot programs shall include delivery of intensive services, which are defined as levels of care 3, 4, or Early Onset within the

During Phase One, the Performing Agency, under the direction of the System Agency’s Program Project Manager (Project Manager), shall administer the School Survey. B. Conduct the School Survey in the winter and spring of the state fiscal year. The Performing Agency shall collect data to be used to produce statewide School Survey aggregate data and reports in the following Fiscal Year . The Performing Agency shall also provide incentives for the participating individual schools selected in the statewide sample. In addition, The Performing Agency shall conduct an over-sample of schools in 28 counties located along the Texas-Mexico border (Special Study Area) concurrent with the School Survey. The counties in the Special Study area are Brewster, Brooks, Cameron, Culberson, Dimmitt, Duval, Edwards, El Paso, Hidalgo, Hudspeth, Xxxx Xxxxx, Xxx Xxxx, Xxx Xxxxx, Xxxxxx, Xxxxxx, Xx Xxxxx, Xxxxxxxx, Pecos, Xxxxxxxx, Xxxxxx, Xxxxx, Xxxxxxx, Xxxxxx, Xxx Xxxxx, Xxxx, Xxxxxxx, Xxxxxx, and Xxxxxx. C. In addition, and in conjunction with the statewide effort, the Performing Agency shall conduct a second over-sample of schools in each of the eleven state planning regions (Regional Study) concurrent with the School Survey for the purpose of providing regional reports in addition to a statewide report. The Performing Agency shall provide Regional Study aggregate data and reports and provide incentives for the participating individual schools in each region. It is possible that some collapsing of regions shall be required if there is insufficient participation. Regional data may also be collapsed if the standard error due to sampling in one or more of the regions is too high. Any collapse of target data will be approved in writing by the System Agency. D. The activities to be performed by the Performing Agency under this Contract detailed in the Tasks below are collectively referred to as the Project. The Project shall include printing of survey instruments and materials; use of incentives for the statewide School Survey and Regional Study recruitment; statewide School Survey and Regional Study administration; and data scanning and processing. The parties acknowledge that this agreement is not a sponsored research agreement or research contract or grant. E. All activities in this Project will be coordinated with the recruitment of schools and students for the Texas Youth Tobacco Survey (YTS). The work on the YTS will be done by the Performing Agency ...
PERFORMING AGENCY RESPONSIBILITIES. Performing Agency will: A. Work with System Agency Birth Defects Epidemiology and Surveillance Branch defects clinical review activities described herein; Each clinical reviewer must meet the minimum qualifications listed below: 1. The clinical reviewer must be a licensed physician in the United States; 2. The clinical reviewer must be an American Board of Medical Genetics and American Board of Medical Genetics [ABMG] Board Certified/Board Eligible Clinical Geneticist who participates in the ABMG Maintenance of Certification program and/or completes twenty (20) hours of Continuing Medical Education 3. The clinical reviewer must have demonstrated experience in birth defects, embryology, genetic disease, and/or syndrome diagnosis. B. Ensure clinical reviewers participate in training as needed on the coding, database system, and procedures used by BDESB; C. Coordinate and ensure clinical review of selected cases abstracted for the BDESB and identification of corresponding birth defect diagnoses and coding as requested by the BDESB Manager, Registry Operations Manager, Senior Scientist, or other BDESB staff in 1. Reviewing electronic copies of birth defect cases meeting BDESB criteria for clinical review; 2. Reviewing diagnosis codes of a random subset of birth defect cases for quality assurance purposes; and 3. Reviewing and providing clinical input for cluster investigations, BDESB studies and special projects; D. Ensure clinical reviewers comment electronically, as necessary, on case abstractions, including but not limited to: 1. Documenting needed changes within each case abstraction; 2. Eliminating a case from the database because it does not meet Birth Defects Registry's case definition; and 3. Notifying the System Agency regional field personnel of needed changes, to request more information, or to request abstraction, as needed; E. expert as requested by the BDESB Manager, Registry Operations Manager, Senior Scientist, or other BDESB staff, including 1. Participation in Birth Defects Registry cluster investigations, periodic BDESB staff meetings, and temporary targeted working groups, testing the BDESB web-based system for collecting and editing data on cases of birth defects, and helping in projects requiring clinical input;

Provide the following services to individuals served by Lubbock State Supported Living Center (LbSSLC): 1) Audiology services to individuals at LbSSLC, in compliance with professional standards established by the Texas State Board of Licensing and Regulation. Diagnostic, rehabilitative, and other services associated with hearing, balance, tinnitus management and related audiological fields. 2) Hearing aid services to include but not limited to modifications and repairs. 3) Provide services in accordance with the following specifications: services provided in accordance with ICF/IDD standards. Contractor's schedule must be mutually agreed upon by Contractor and Center. 4) Provide at least one clinic per month; Services to include Audiological Annual Exams, any cleanings, and adjustments, as necessary. 5) Interact with staff, other contractors or consultants of the facility in a cooperative manner and will consult with such persons regarding services provided under this agreement as necessary.

Deliver tobacco cessation CHWI CEU trainings. 4. Develop dissemination tobacco cessation resources (i.e., flipcharts, resource folders) to complement CHW/CHWI trainings. 5. Conduct an evaluation on the effectiveness of the CHW trainings.
