Grievance Contents. All grievances (HR Form-23) must contain the following information to be considered and must be filed using the grievance form mutually agreed upon by both parties. The grievance will stipulate the exact article and section of this agreement to which the grievance pertains and the precise remedy for the grievance. 1. Aggrieved employee’s name and signature. 2. Aggrieved employee’s classification. 3. Date grievance was first discussed with his supervisor. 4. Date grievance was filed in writing. 5. Date and time grievance occurred. 6. The location where the grievance occurred. 7. A description of the incident(s) giving rise to the grievance. 8. Specific articles and sections of the Agreement. 9. Xxxxxxx remedy to resolve the grievance.
