Health and Safety Protocols. For Contracts to provide mental health, behavioral health or addiction services to clients of the Division of Mental Health and Addiction Services, consistent with P.L. 2021, Chapter 1, the Provider Agency shall make a good faith effort to comply with all applicable Federal and State COVID-19 health and safety protocols to adequately ensure the safety of the Provider Agency’s employees and service recipients. This section shall expire on the 366th day following the end of the public health emergency and state of emergency declared by Governor Xxxxxx X. Xxxxxx in Executive Order No. 103 of 2020, as subsequently extended by executive order.
Health and Safety Protocols. Collaborator will comply with all Client health and safety guidelines and protocols (including, without limitation, those relating to COVID-19) as may be adopted by Client. Client may in its sole discretion revise such guidelines, protocols, and policies from time to time. Collaborator acknowledges that Client may require Collaborator to take certain actions as a result of city, county, or other government directives or guidelines, and that Client may implement, at its discretion, requirements that go beyond binding public health directives from the government.

Health and Safety Protocols. 1.0 District-wide health and safety protocols will be designed to comply with guidance of all applicable public health agencies. Strict compliance with all relevant District safety and health rules will be an essential function of each employee’s job. Failure to comply with COVID-19 vaccination requirements will lead to non-disciplinary dismissal, in-alignment with article 2.0. Employees who have an approved medical or religious exemption from receiving the vaccine will not be subject to non-disciplinary dismissal. Failure to comply with other health and safety protocols, including wearing appropriate PPE, as defined by state Department of Labor & Industries (L&I) guidelines, and communicated by the building COVID supervisor, may lead to progressive discipline, up to termination.
Health and Safety Protocols. 17.1 Seafarer agrees that he/she will comply with all applicable health and safety protocols. This includes, but is not limited to, health and safety protocols implemented to comply with applicable Flag State Laws and regulations, including any mandatory quarantine or isolation periods beginning immediately when Seafarer embarks on his/her assigned vessel when Seafarer is not permitted to work in their assigned position (“Commencement Period”).

You must supervise the entry lounge at any time the Facility doors are open. These doors must remain closed at all other times. • You will enter and exit the Facility through the main door. • You will enter and exit the Space using only the main set of doors off the lobby. • There are limited security cameras in the Facility. The cameras capture view of the front door, hallways/stairs, and back hallway/side door. COVID-19-Specific Policies
Health and Safety Protocols. All employees representing Exhibitor’s company must adhere to all health and safety protocols set forth by LeadingAge for this event. After a warning from LeadingAge staff, any representative of the Exhibitor company not adhering to health and safety protocols will be asked to leave the event. No refunds will be provided.
Health and Safety Protocols. ENVIRON has developed a Site Health and Safety Plan (HASP), provided in Appendix E, for sampling and remedial activities at the Site that meets the requirements of 29 CFR 1910.120. Based on the nature of soil contamination detected to date, and the confirmed absence of dust/particulates at levels of potential concern during the prior site work, future soil and groundwater sampling activities are projected to be completed with Level D personal protective equipment (PPE). However, the HASP also specifies PID monitoring during the sampling activities, and potential PPE upgrades based on PID readings. As recommended in DER-10, Sun Chemical and ENVIRON evaluated whether a Community Air Monitoring Program (CAMP) would be required as a component of the proposed RIWP. As indicated above, the site has no known VOC soil contamination. Further, no elevated PID readings were recorded during the IRM program. Last, as indicated above, during the IRM program Sun Chemical completed a CAMP but identified no particulate readings on a 15-minute time weighted average basis that exceeded the NYSDOH suggested limit of 100 μg/m3. ENVIRON also did not observe any visible dust migrating from the work areas. These data and observations therefore indicate that release of dust/soil particulates during planned RI activities is not of concern. In light of those prior air monitoring results and the limited nature of ground disturbance planned, Sun Chemical proposes that a CAMP not be required during implementation of the RIWP. Based on prior discussions regarding this issue, the parties agreed that a CAMP is not required.

I certify that I will wear a mask in accordance with Oregon’s mask mandate which requires people in Oregon to wear masks in all indoor public spaces and in most public outdoor settings regardless of vaccination status where physical distancing is not possible. I certify that I will follow Lane County Public Health/ Oregon Health Authority guidance for COVID-19 and other communicable diseases. Student-centered events I certify that the event I am renting Bethel district space for IS NOT a student event, meaning that groups of students will not be gathering as part of this event. That this event is not taking place during school hours OR when students and staff are typically present in the building; and that the space is available.

In connection with the foregoing, I, on my behalf or on behalf of my child, acknowledge and agree to comply with (1) all relevant policies and protocols issued by the Cardinals and/or the Facility, including, without limitation, any policies and protocols regarding security, bags, fan conduct and health and safety, currently available at xxx.xxxxxxxxx.xxx/xxxxxx-xxx-xxxxxx-xxxxxxxx (collectively, “Health and Safety Policies”), all of which, due to the evolving nature of the COVID-19 pandemic, may continue to be updated from time to time until the time of the Event, and (2) all current guidance of the Centers for Disease Control and Prevention and all applicable laws and policies of federal, state, city and local authorities. Cardinals have the right to deny admission to or eject any person who the Cardinals or such designees determine, in their sole discretion, poses a risk to the health or safety of others and/or whose conduct violates Health and Safety Policies. I assume all risk of my child not being admitted to or ejected from the Facility as a result of a violation of Health and Safety Policies.
