Vaccination Requirements. Contractor Staff who engage in work while physically present at a building, facility, jobsite, project site, unit, or other defined area owned, leased, occupied by, or controlled by DSHS must be fully vaccinated against COVID-19, unless a DSHS- approved exception exists, or Contractor has granted the Staff a DSHS-approved disability or religious accommodation. Contractor and Contractor Staff shall provide proof of such vaccination or accommodation upon request by DSHS. Contractor shall cooperate with any investigation or inquiry DSHS makes into Contractor’s compliance with these requirements, including providing information and records to DSHS upon request, other than information or records that Contractor is prohibited by law from disclosing. Contractor may request a copy of the requirements for full vaccination, acceptable methods of proof and DSHS-approved exceptions list from the DSHS Contract Manager listed on page one of this Contract.

Vaccination Requirements. Contractor shall abide by the vaccination requirements of Governor Xxx Xxxxxx’x Proclamation 21-14 and all subsequent amendments. After October 18, 2021 Contractor Staff who are reasonably likely or contractually obligated to engage in work while physically present at a building, facility, jobsite, project site, unit, or other defined area owned, leased, occupied by, or controlled by a State Agency, an operator of an Educational Setting, or an operator of a Health Care Setting as defined in the Proclamation must be fully vaccinated against COVID-19 unless they have been granted a valid disability or religious accommodation by Contractor. Contractor shall obtain a copy of, or visually observe proof of full vaccination against COVID-19 for all Staff who are subject to the vaccination requirement in the Governor’s Order. Contractor shall follow the requirements for granting disability and religious accommodations to Contractor’s Staff that apply to State Agencies under the Governor’s Order. Contractor and Contractor Staff shall provide proof of such vaccination or accommodation upon request by DSHS. Contractor shall cooperate with any investigation or inquiry DSHS makes into the employer’s compliance with these requirements, including by providing information and records upon request, except any information or records that the employer is prohibited by law from disclosing.
Vaccination Requirements. Category A workers are required to meet and maintain the vaccination criteria (as per the current NSW Health and STARTTS policies) including annual influenza vaccinations. Two doses of an approved COVID-19 vaccine will be required to provide a record of your COVID-19 vaccination status. Annual Influenza vaccinations are mandatory. Vaccination and/or assessment for several other transmissible diseases are also required. Category B workers must have two doses of an approved COVID-19 vaccine and must also provide COVID-19 vaccination evidence if successful. (The annual influenza vaccination as well as vaccination for other transmissible diseases are strongly encouraged but not mandatory). Should you not be able to comply with vaccination requirements before your commencement date, the offer of employment may be withdrawn.

This requirement extends to partners’ workers (staff and volunteers) who will be working on Highline Public Schools’ property. Vaccination Requirements. Students wishing to participate in a ServiceIQ Readymade (fully supported) Gateway programme which has an in-workplace component, and where the employer requires it, must be fully COVID-19 vaccinated. Vaccination Requirements. COVID-19 continues as an ongoing and present threat in Washington State. The measures we have taken together as Washingtonians over the past several months have made a difference and have altered the course of the pandemic in fundamental ways. It is the duty of every employer to protect the health and safety of employees by establishing and maintaining a healthy and safe work environment and by requiring all employees to comply with health and safety measures. As a result of the above noted situation, and consistent with the Governor’s proclamation 21-14.3 all employees of the Office of the Secretary of State are now required to become fully vaccinated or have an approved accommodation, as provided in this Memorandum of Understanding (MOU). In recognition of the above, the parties agree to the following: All employees will take the necessary steps to be fully vaccinated by February 25, 2022 or be approved for a medical or religious accommodation, unless otherwise authorized under this MOU. The definition of fully vaccinated may in the future include FDA-approved booster shots. The parties agree to meet within thirty (30) calendar days of any announcement that booster shots will become a requirement for continued employment and bargain the impacts in good faith to achieve the health and safety goal.
Vaccination Requirements. Except as otherwise provided below, to the extent this Contract requires contractor, employees of contractor, or any subcontractor to perform any type of in-person work, including, but not limited to, the practice of medicine and health care, consulting, training, construction, maintenance, and vendor services, on property owned or leased in any capacity by CSN, any and all employees of contractor and each subcontractor who will perform such in-person work must have completed a COVID-19 vaccination series before they will be permitted to perform such in-person work. By executing this Contract, contractor represents and warrants that it is solely responsible for ensuring full compliance with this provision and such compliance shall occur before any contractor or subcontractor employee covered by this provision enters CSN property. Failure to comply with the foregoing shall constitute a material breach of this Contract, which, if not fully cured within thirty (30) days after CSN’s written notice of breach, entitles CSN to terminate the Contract. No inability, delay, or failure to comply with this provision shall be deemed a valid excuse orjustification for contractor to fail to timely complete the work specified herein. A “completed COVID-19 vaccination series” is given the same meaning as in Title 2, Chapter 12, Section 4(C) of the Nevada System of Higher Education Code and is defined below and made a part of this Contract. Within 72 hours of CSN’s written request to contractor, contractor shall provide to CSN records, acceptable to CSN in its sole discretion, of a completed COVID-19 vaccination series for any and all employees of contractor and each subcontractor performing such in-person work. Notwithstanding anything to the contrary contained above, the foregoing vaccination requirements shall not apply to any employee of contractor/subcontractor working on a construction project which is not reasonably accessible to CSN employees or students, or members of the public, provided, however, that such employees shall comply with all directives of the Centers for Disease Control and Prevention, Occupational Health and Safety Administration, and the state of Nevada concerning COVID-19. Vaccination Requirements. 21.1. Centers must follow the COVID-19 vaccination requirements in the Governor’s Proclamation 21-14, and any subsequent versions of the proclamation at the time of service, and Washington State Department of Heath mandates. This includes managing Center staff’s vaccination, exemption, and accommodation requirements and processes.
Vaccination Requirements. 3.01 The HPA requires all employees, tenants, contractors whilst working in the port or other business partners accessing port infrastructure to be fully vaccinated with a COVID-19 vaccine series by the deadlines set out in Article 3.02 of this Policy. This requirement is subject only to the human rights exemptions described in Articles 3.08 of this Policy.
Vaccination Requirements. All dogs boarding or attending daycare at PEK must be current on their Rabies, Distemper/Parvo, and Bordetella vaccinations. Dogs receiving grooming services only are required to be current on their Rabies and Distemper/Parvo vaccinations; the Bordetella vaccine is not required, but is recommended. The oral and intranasal forms of the Bordetella vaccine contain a live virus which dogs can shed into the environment for a brief period following vaccination. For this reason, dogs getting the intranasal or oral vaccine must have it done at least 7 days prior to any boarding or daycare reservation at PEK. If the vaccination must be done less than 7 days before a reservation, PEK will only accept dogs who have received the injected (killed virus) form of the vaccine. The Client understands that valid and up-to-date vaccination records must be provided in order for their dog to receive services at PEK. PEK reserves the right to refuse service if any of the above requirements are not met.
