Health Plan Affidavit. A. Michigan State University requires that spouses or MSU Recognized Other Eligible Individuals (OEI) eligible for health plan coverage through another employer with an annual employee premium cost as determined by MSU must enroll in that employer’s coverage in order to enroll/maintain coverage through MSU. B. A Health Plan Affidavit must be completed by employees at the time of hire and each year during open enrollment for their spouse or an Other Eligible Individual to be enrolled in an MSU health plan. C. Dependents 1. Covered on the health/prescription insurance until the end of the calendar year they turn age 26. 2. Eligible dependents include son, daughter, stepchild, adopted child, or eligible ▇▇▇▇▇▇ child (which only goes to age 18). 3. Parents or other relatives over age 26 can be covered, provided IRS dependent guidelines are met, by purchasing a Sponsored Dependent Rider through payroll deduction.
