Prescription Insurance. The Board shall pay the full premium of prescription insurance for each employee and dependent(s), subject to a $20.00 co-pay for brand name prescriptions and a $10.00 co-pay for generic prescriptions. There co-pays shall apply to mail order/mail-in prescriptions.

The Board shall provide a Family Coverage program of prescription insurance based upon the following co-pay amounts: Nonspecialty Retail Home Delivery (90-day supply) Generic $10 $20 Formulary $30 $60 Non-Formulary $60 $120 Specialty $100 $200 Step Therapy, Preferred Specialty Management, and Prior Authorization with additions shall be in effect. The Prescription insurance annual out-of-pocket maximum shall be Five Thousand Dollars ($5,000.00).
Prescription Insurance. 31.1 The Xxxxxxx Board of Education will provide prescription insurance coverage for each member of the bargaining unit through the HESE Wellness Plan effective August 1, 2015.
Prescription Insurance. 14:9.1 During the leave, if the teacher recovers and is certified by his or her physician as being able to return to a Board-offered vacancy and refuses to do so, or if the employee accepts employment elsewhere, all Board payments (the benefits noted above) will cease.
Prescription Insurance. An Employee retiring on or after January 1, 2004 is entitled to continue in the Township’s Prescription Plan as well as the eligible immediate dependents at time of retirement at Retiree’s expense. The Township reserves the right to bill for an additional reimbursement in the succeeding year of up to twenty-five percent (25%) of premium whenever the total claims of the prior year exceed the annual premium of prior year by up to twenty-five percent (25%). For example: Annual premium for year X is $100.00; total claims for year X are $119.00; and annual premium for year Y is $125.00. Retiree in year Y would be billed $125.00 for current year plus $19.00 ($119.00 divided by 100 = 119% x 100.00 = $119.00 - $100.00 = $19.00) for year X’s utilization in excess of premiums.
Prescription Insurance. Subject to the provisions of Section 4.05 below, the Board will provide prescription insurance coverage for each member of the bargaining unit. Employees may elect to receive twenty-five percent (25%) of the current single premium in lieu of participating in the prescription insurance program upon providing proof of other insurance coverage that is not through the Health Insurance Marketplace. Payment is to be made the first pay in July. Employees are not eligible for this payment in lieu if they are covered by insurance provided by the Board or any other member of the health insurance consortium of which the District is a member.

The Employer shall make available to all employees covered by this Agreement, who are enrolled under one of the Employer's health insurance plans, a $7.00 co-pay for prescription drugs. The Employer has contracted with a managed pharmacy drug benefit program for a three-tier prescription benefit at $7/$15/$35.

The claim must be filed under the medical plan for consideration by the carrier.
Prescription Insurance. SECTION 1. Effective upon the date of ratification of the within agreement, the Prescription Plan provided shall be the NJ DIRECT 15 Plan maintained by the SHBP or a plan that is substantially similar or greater to the provisions of the NJ DIRECT 15 Plan. Schedule B governs the obligations of contributions by employees.

The Township shall provide to each regular, full-time, permanent Employee and the dependents of the immediate family a five dollar ($5.00) generic/ten dollar ($10.00) brand Co-pay, No Contraceptives Prescription Plan. Employees and/or dependents taking prescription drugs for a long-term, ongoing medical condition must obtain this maintenance medication through a mail service program with Prescription Drug Service, Inc., a subsidiary of our prescription drug carrier. The co-pay applies to each time you purchase a supply of drugs through the mail service program.
