Information available to You. You may request from Us:
a) a copy of the Small Use Customer Code;
b) information on the Fees and Contract Prices applicable to the Contract and, other prices, if any, available to You and the time of Your request;
c) historical billing data;
d) information on any concessions applicable to You;
e) information on service standard payments available to You from Us or the Network Operator;
f) information on energy efficiency;
g) information on any Network Access Tariffs applicable to Us;
h) information relating to the distribution of electricity or metering; or
i) the Contract. We will provide You with the relevant information in writing (if so requested) within 8 Business Days of Your request. Unless the Law requires Us to provide the information free of charge, We can ask You to pay a reasonable charge for the information.
Information available to You. You may request from Us:
(a) a copy of the Small Use Customer Code;
(b) information on the Fees and Contract Prices applicable to the Contract and, other prices, if any, available to You and the time of Your request;
(c) historical billing data;
(d) information on any concessions applicable to You;
(e) information on service standard payments available to You from Us or the Network Operator;
(f) information on energy efficiency;
(g) information on any Network Access Tariffs applicable to Us;
(h) information relating to the distribution of electricity or metering; or
Information available to You. If you request it, we will supply you with: [a] a copy of the Code; and [b] a copy of the fees and prices payable under the contract and, if any, other prices that may be available to you at the time of your request; and [c] a copy of the contract; and [d] information on the types of concessions available if you are a residential customer; and [e] general information on energy efficiency, including typical running costs of major domestic appliances; and [f] information relating to the distribution of electricity. Any information requested by you will be provided within eight [8] business days of receipt of the request and in writing [if so requested]. Unless a law requires us to provide the information free of charge, we can ask you to pay a reasonable charge.
Information available to You. If you request it we will supply you with a copy of: [a] our customer charter; or [b] the Energy Coordination [Customer Contracts] Regulations 2004 or any relevant code; or [c] the fees and prices payable under the contract; or [d] information about how you can use energy more efficiently; or [e] your billing data for the previous 2 years [you may also request billing data for the period prior to this time subject to a reasonable charge]; or [f] information about government assistance programs or financial counselling services [see clause 7.4 above]; or [g] information on the types of concessions available to you and contact details of the organisation responsible for administering those concessions; or [h] information about our complaints handling process and how to contact the Energy Ombudsman if you are not satisfied with our handling of your complaint [see clause 12 below]; or [i] our privacy policy; or [j] a copy of this contract; or [k] a combination of any or all of the above. Unless a law or our trading licence requires us to provide the information free of charge, we can ask you to pay a reasonable charge.
Information available to You. If you request it we will supply you with a copy of: [a] the Energy Coordination [Customer Contracts] Regulations 2004 or any relevant code ; or [b] the fees and prices payable under the contract; or [c] information about how you can use energy more efficiently; or [d] your billing data for the previous two [2] years [you may also request billing data for the period prior to this time subject to a reasonable charge]; or [e] information about Government assistance programs or financial counselling services [see clause 7.4 above]; or [f] information on the types of concessions available to you and contact details of the organisation responsible for administering those concessions; or [g] information about our complaints handling process and how to contact the gas industry ombudsman if you are not satisfied with our handling of your complaint [see clause 12 below]; or [h] a combination of any or all of the above. Unless a law or our trading licence requires us to provide the information free of charge, we can ask you to pay a reasonable charge.
Information available to You. Even though the purpose of this agreement is to establish limits on when and how information and records related to the therapy will be disclosed to you, Provider recognizes the importance of disclosing some information and/or records to you about the Minor Client’s status and progress in therapy. Therefore, Provider will give you a periodic summary of the therapy that includes, in Provider’s professional judgment, information and/or records that will sufficiently apprise you of the Minor Client’s status and progress without posing an increased risk of harm to the Minor Client and/or others by circumventing the terms of this agreement. In addition to periodic summaries, Provider will notify you of the need for therapy or other professional services for the Minor Client if, in Provider’s professional judgment, notice is necessary to avoid jeopardizing the health of the Minor Client.
Information available to You. For additional information, you should contact the United States Environmental Protection Agency (“EPA”), applicable state agencies, or other governmental authorities. The EPA web site contains information and publications regarding mold and other biological pollutants that may be of interest to you. For example, see “Biological Pollutants in Your Home” and “Mold Resources” on the EPA web site (xxx.xxx.xxx). Additional mold-related information is available on the Centers for Disease Control and Prevention web site (xxx.xxx.xxx).

If you request it, we will supply you with: [a] a copy of the Code ; and [b] a copy of the fees and prices payable under the contract and, if any, other prices that may be available to you at the time of your request; and [c] a copy of the contract; and [d] information on the types of concessions available if you are a [e] general information on energy efficiency, how you may arrange for an energy efficiency audit at your supply address and the typical running costs of major domestic appliances; and [f] information relating to the distribution of electricity. Unless a law requires us to provide the information free of charge, we can ask you to pay a reasonable charge.
