Init. Buyer agrees to take the dog to a veterinarian of his/her choice within seventy-two (72) hours or (3 business M-F days) of delivery of the dog. A full health check including stool sample must be done. If, within that 72 hour period, a veterinarian finds the dog to be unsound, unhealthy, or possessing a serious genetic defect, then a statement in writing must be provided to the Breeder/Seller by that Breeder/Seller’s veterinarian of alleged defects, Buyer may return the dog, registration and all of his/her effects to the Breeder/Seller, and receive a full credit of the purchase price towards a new puppy. Replacement puppy may be given from an upcoming litter of breeders choice within 11 months. All transportation costs involved are the responsibility of the Buyer. The puppy in question must be returned to the Seller within 72 Hours of the vet appointment. If for any reason the puppy would need to be replaced under the terms of this entire contract, buyer MUST show proof of initial vet visit dated within 72 hours of pickup or (3 business M-F days). If no proof of THIS vet visit is furnished at time of any replacement, contract will be null and void.
Init. Buyer agrees that the Breeder/Seller’s kennel name suffix Von Mitternadt is the end of the dog’s registered name on the Registration certificate. Buyer also agrees to use the litter letter as the FIRST letter in the registered name on the puppy. If buyer does not follow this registration procedure this contract will be considered NULL & VOID. Buyer agrees to register transfer of ownership of the dog with AKC within sixty (60) days of receiving the AKC registration papers and that this transfer will be recorded in the name of the party listed on this contract. The buyer also agrees that this dog will not be registered with any other registration service other than the American Kennel Club.
Init photograph/video me and use such photograph(s)/video(s) in all forms of media, for any and all promotional purposes including advertising, display, audiovisual, exhibition or editorial use. I understand that I will not receive any payments for my time or royalties for the use of image.
Init. Indoor motorized chairs and scooters: We do not allow motorized chairs or scooters for indoor use at Xxxxxx Manor. Should a resident decide to acquire an outdoor scooter they must be assessed by an Occupational Therapist arranged for by VIHA, prior to using the scooter at Xxxxxx Manor.
Init. 1. Telephone jacks are provided in each suite. The Tenant is required to have his/her own telephone. The Tenants’ telephone number must be given to the Operator. The Tenant must make his/her own arrangements for installation, payment, service, and removal of other special utility service(s). The Operator does not accept responsibility for any charges.
Init. 6. As our curriculum emphasizes “leadership, citizenship and scholarship,” we expect cadets to abide by the rules established by Weld County School District 6 as well as those regulations we teach in MCJROTC. We accept honest mistakes, but expect cadets behavior in school and all classes to be exemplary. Other teachers know our expectations and support us as well. INIT.
