Life. The City shall pay one hundred percent (100%) of the monthly premium for a $50,000 life insurance policy with accidental death and dismemberment, for each member of the bargaining unit.
Life. The University shall continue to make available a group life insurance program. The program shall be the same basic program as applicable to the University's other staff members. The minimum life and accidental death and dismemberment insurance amount will be an amount equal to base salary.
Life. The College shall provide group life insurance equal to two (2) times the annual salary, rounded off to the next $1,000 for each eligible employee, but not to exceed $120,000.
Life threatening is defined as: the patient is at immediate risk of death from the AE as it occurs.
Life. The City shall provide twenty thousand dollars ($20,000) life insurance protection with double indemnity for the accidental death of a supervisor of the bargaining unit.
Life. Reinsurance under this Agreement is on a Yearly Renewable Term basis. The reinsurance benefit will be determined in the manner described below.
Life. The County agrees to provide twenty thousand dollars ($20,000) in life insurance for employees.
Life. The Board shall select and pay the full premium cost for a term life insurance plan in the face value of thirty thousand dollars ($30,000) for each full-time eligible employee.
Life. For Life insurance claims, the Reinsurer shall pay its share of death benefits in a lump sum regardless of the form of claim settlement by the Company.
