Life Insurance Protection. A $5,000.00 life insurance policy shall be provided each employee. Life insurance shall remain in full force and effect for employees on paid sick leave or during an approved unpaid leave of absence of less than thirty calendar days.

Life Insurance Protection. 34.01 The Board shall provide without cost to the K-12 employees who work a scheduled twenty (20) or more hours per week (childcare employees more than twenty (20) hours per week), group life insurance protection in the amount of twenty five thousand dollars ($25,000). For those K-12 employees who work less than twenty (20) hours per week (childcare employees twenty (20) hours per week or less), the Board shall provide group life insurance in the amount of ten thousand dollars ($10,000). Each employee is entitled to only one (1) policy regardless of combined hours. It is the employee’s responsibility to process with administration the required participation forms. Life Insurance Protection. If an employee has life insurance, mortgage repayment insurance or disability insurance policies which have an exclusion clause,and the employee dies or is disabled in the normal course of duty and in circumstances in which the employeewas required, or could normally be expected to perform the duties which result in application of the exclusion clause, theDepartment will make an ex gratia payment equivalent to the sum assuredplus bonuses.

To maintain this cover the company agrees to pay the amounts every week for each employee. In the event the company does not maintain the above policy, the company will be liable in full to pay equivalent benefits to an employee who meets eligibility criteria as set out in the policy document.

  • HEALTH AND LIFE INSURANCE The Sheriff will continue to offer to employees of this bargaining unit the same health and life insurance programs being offered to other Sheriff’s Office employees. Due to the changing nature of the health insurance market, the Sheriff retains the right during the term of this Agreement to develop plan changes or to change carriers in order to reduce costs or for other reasons. Changes to the level and types of benefits shall be subject to bargaining as provided by law. Rates paid by the employee will be established by the Sheriff.

  • Group Life Insurance The Hospital shall contribute one hundred percent (100%) toward the monthly premium of HOOGLIP or other equivalent group life insurance plan in effect for eligible full-time employees in the active employ of the Hospital on the eligibility conditions set out in the existing Agreements.

  • Insurance Programs Upon proper application and acceptance for enrollment by the appropriate insurance underwriter, policyholder, or third-party administrator, the Board shall make premium payments on behalf of the Administrator and his/her eligible dependents for the insurance programs, subject to possible modification as stated in ¶ 10 below.

During the life of this Agreement, the basic life insurance benefit made available to Faculty members shall be calculated as 3 times base annual earnings, rounded to the next highest $1,000, but not more than $225,000. A separate additional benefit up to the amount of the life insurance will be paid for accidental death and dismemberment, or loss of sight. The amount of Life and Accidental Death and Dismemberment/Loss of Sight benefits will be reduced to 65% at age 65, and further reduced (from the original insurance amount) as follows: to 50% at age 70, and 35% at age 75. Basic life insurance and AD&D benefits will be provided with no employee contributions.
