Medical, Group Life Insurance and Dental Plans. 28.03 Medical Plan All regular employees who are eligible under the terms of the Medical Services Plan and the mutually accepted Extended Health Benefits Plan will, as a condition of employment, participate in the Plan from the first calendar day of the month following commencement of employment. All temporary employees appointed for a minimum of 3 months half-time or more are eligible to apply for coverage under the Medical Services Plan and the Extended Health Benefits Plan.

Medical, Group Life Insurance and Dental Plans. 28.03 Extended Health Plan The parties are agreed, subject to similar agreement with CUPE Local 917, to the below changes to the plan: • Effective July 1, 2020, lifetime maximum, no limit and Dental plan A to 90% coverage. • Effective April 1, 2021, Dental plan B to 70% and Dental plan C to 60%.   • Optional Group Life Insurance Subject to the provisions of the Plan, eligible employees shall be entitled to purchase optional Group Life Insurance coverage in units of ten thousand dollars ($10,000) up to a maximum of two hundred and fifty thousand dollars ($250,000). The employee shall pay one hundred percent (100%) of the premiums for the optional coverage.

  • Retiree Life Insurance Employees who retire under the Monroe County Employees' Retirement System shall be eligible for $4,000.00 term life insurance. All employees hired by the Employer on or after October 1, 2007 shall not be eligible for Retiree Life Insurance.

  • Dependent Life Insurance In the event of the death of your spouse or dependent child from any cause whatsoever, while you and your dependents are insured under the plan, the insurance company will pay you $10,000 in respect of your spouse and $5,000 in respect of each insured dependent child. This applies to those employees with family health coverage only.

  • HEALTH AND LIFE INSURANCE The Sheriff will continue to offer to employees of this bargaining unit the same health and life insurance programs being offered to other Sheriff’s Office employees. Due to the changing nature of the health insurance market, the Sheriff retains the right during the term of this Agreement to develop plan changes or to change carriers in order to reduce costs or for other reasons. Changes to the level and types of benefits shall be subject to bargaining as provided by law. Rates paid by the employee will be established by the Sheriff.
