Extended Health Benefits Plan. The Board shall provide the Union with a copy of all master benefit plans and shall provide annually the union a copy of the financial/actuarial statements for all benefit plans. [Not applicable for the Provincial Extended Health Benefit Plan. See Article B.11.2.]
Extended Health Benefits Plan. (a) The University will maintain an extended health benefits plan for all eligible employees. The operation of this plan will be governed solely by the master agreement between the University and the plan carrier.
(b) Continuing employees who are covered by a basic medical plan are eligible for coverage under this plan commencing on first day of the month following employment in a continuing position.
(c) The University will pay the required premium for this plan.
(d) Eligible expenses will be subject to an annual twenty-five (25) dollars deductible. After the deductible has been satisfied, reimbursement will be eighty (80) percent. Corrective vision reimbursement is limited to a maximum payable of two hundred (200) dollars in any twenty-four (24) month period. Eligible expenses will include up to five hundred (500) dollars per year per member or dependent of fees for a podiatrist, registered clinical psychologist, or chiropractor. The lifetime maximum aggregate amount payable by the plan carrier is limited to one million (1,000,000.00) dollars per person for group extended health care benefits under all agreements with the carrier.
Extended Health Benefits Plan. The contract currently held by the Companies signatory to this Agreement and the Medical Services Association to provide Extended Health Benefits Plan to employees classified herein and who indicate their desire to be covered by the plan, shall remain in effect for the duration of this Agreement.
Extended Health Benefits Plan. (a) Regular Full-Time Employees and Temporary Full-Time Employees who have completed the probationary period of six (6) months shall be entitled to enroll and to be covered effective the first day of the calendar month following such period.
(b) A spouse, dependent children to age 21, and dependent children over age 21 in full-time attendance at a recognized school or college, shall be covered as employee dependents.
(c) The plan shall reimburse employees at the rate of eighty percent (80%) of insured eligible expenses after a deductible of $25 (effective 2004 January 01, $50; effective 2005 January 01, $100) per family per year.
(d) The Plan has a lifetime maximum of one million dollars per person and includes, among other benefits, coverage for eye exams, effective 2004 January 01, to a maximum payable of $75 per person every two (2) years, vision care with a maximum payable of $300.00 per person in a twenty-four (24) month period, laser eye surgery, effective 2004 January 01, with a lifetime maximum payable of $500 per eye, hearing aids (coverage in the amount of $700 (effective 2004 January 01, $1,000) over a five-year (60 months) time frame), diabetic equipment and supplies, orthopedic shoes, ostomy, clinical psychologist (coverage in the amount of $600 per year), and the Nicotine Patch with a $350 lifetime maximum, all subject to the provisions of the Plan.
(e) The City shall pay one hundred percent (100%) of the premiums required for the Plan.
Extended Health Benefits Plan. 14.11.1 The College will contribute 100% of the premium for Extended Health Benefit coverage according to the terms of the contract with the insuring company for those temporary and casual employees who have elected benefit coverage and permanent employees who elect to participate.
14.11.2 Benefits are payable at the rate of 95% of in-province eligible expenses in excess of the deductible and 100% of emergency out of province eligible expenses in excess of the deductible. The deductible is $25.00 per year.
14.11.3 The Extended Health Benefit Plan will include, among other benefits:
a) Optical care insurance to a maximum of $300.00 payable every 2 years, according to the terms of the contract with the insuring company;
b) Coverage for orthotics to a maximum of $200.00 payable each year;
c) Registered clinical psychologist coverage to a maximum of $600.00 payable each year;
d) Hearing aid coverage to a lifetime maximum payable of $600.00 every 5 years;
e) Total lifetime coverage will be unlimited;
f) Medical Travel Referral benefit will be improved by increasing the per diem benefit for service and supplies not covered by the Medical Services Plan of B.C. to $125 per day for up to 50 days;
g) Health and welfare benefits coverage will cease on the day that an employee’s employment terminates.
Extended Health Benefits Plan. The Board shall pay 100% of the cost of the premiums for the Provincial Extended Health Benefit Plan. Participation in the plan is compulsory.

10.3.4 The Plan has a lifetime maximum of one million dollars per person and includes, among other benefits, coverage for eye exams to a maximum payable of $100 per person every two (2) years, vision care with a maximum payable of $450 per person in a twenty-four (24) month period, laser eye surgery with a lifetime maximum payable of $500 per eye, hearing aids (coverage in the amount of $1,000 over a five-year (60 months) time frame), diabetic equipment and supplies, orthopedic shoes, ostomy, clinical psychologist (coverage in the amount of $600 per year), and the Nicotine Patch with a $350 lifetime maximum, all subject to the provisions of the Plan.
10.3.5 The maximum physiotherapist and registered massage therapist combined coverage shall be $1250 per year.
10.3.6 Effective 2017 January 01, the Plan shall reimburse drug expenses based upon mandatory generic pricing, except where the employee’s physician provides confirmation of no generic substitution on the prescription.
10.3.7 The City shall pay one hundred percent (100%) of the premiums required for the Plan.
Extended Health Benefits Plan. Each full‐time employee and their eligible dependants shall be entitled to coverage under the Extended Health Benefits Plan following three (3) months of continuous employment. The Extended Health Benefits Plan coverage is subject to the provisions of the Plan. The Extended Health Care Plan has a lifetime maximum of one million dollars ($1,000,000.00) per person, an annual deductible of one hundred dollars ($100.00) (effective 2015 January 01, one hundred and twenty‐five dollars ($125.00)), and includes, among other benefits, coverage for vision care with a maximum payable of four hundred and fifty dollars ($450) per person in a twenty‐four (24) month period, eye exams payable at one hundred dollars ($100) per person in a twenty‐four (24) month period, hearing aids, diabetic equipment and supplies, orthopedic shoes, and ostomy. The plan also includes total coverage of up to one thousand five hundred dollars ($1,500) payable per person per calendar year for the services of any one or any combination of the following practitioners: Psychologist, Chiropractor, Naturopath, Physiotherapist, Massage Therapist, Acupuncturist, Speech Language Pathologist and Podiatrist, all subject to the provisions of the Plan.
Extended Health Benefits Plan. The employer shall pay 100% of the premiums for eligible employees and their dependents. Extended Health Benefits Plan. The extended health benefits plan covers major medical expenses incurred for treatments that are not covered by the provincial Medicare plan or any other government-sponsored program. Coverage applies to eligible regular Employees and their qualified dependents under the age of nineteen (19) years, or under the age of twenty-five (25) years provided the child is wholly dependent on the Employee and is in full-time attendance at a recognized post-secondary educational institution. Effective October 1, 2013, Direct pay drug card is available on the following conditions: - drugs reimbursed at 80 %; - mandatory generic substitution; - maximum dispensing fee of $9.25. To be eligible for reimbursement, medical expenses must be medically necessary for treatment of sickness or injury and recommended by a licensed physician, and must be reasonable and customary.
